Postpartum pulmonary thromboembolism in a patient homozygous for the factor V leiden gene mutation

Özet
Bu makalede 19. gestasyonel haftada pulmoner tromboembolizm tanısı konulan 20 yaşında primagravid bir hasta sunulmuştur. 19. gebelik haftasında rutin gebelik incelemesi sırasında hemoglobin düzeyinin 6 gr/dL olarak saptanması üzerine ileri hematolojik inceleme için Ankara Üniversitesi Tıp Fakültesi'ne sevk edilen hastanın ultrasonografisinde 16. gebelik haftası ile uyumlu intrauterin ölü fetus saptanmış ve 100mg misoprostol uygulanarak gebelik terminasyonu gerçekleştirilmiştir. Terminasyon sonrası 2. günde nefes darlığı ve sırt ağrısı yakınmaları başlayan hastaya, ventilasyon perfüzyon sintigrafisi ve bilgisayarlı tomografi angiografi ile sağ akciğer bazal segmentinde pulmoner thromboembolizm tanısı konulmuştur. Derin bacak venlerinin renkli Doppler incelemesi normal olarak saptanmıştır. Koagulasyon profili tamamen normal olarak saptanan hastanın DNA polimeraz reaksiyon amplifikasyonu ile faktor V Leiden mutasyonu açısından homozigot olduğu ortaya çıkarılmıştır.
Abstract
In this paper we report a 20 years old primigravid woman suffered from pulmonary thromboembolism on 19th gestational week. During routine pregnancy examinations the hemoglobin value was detected to be quite low and patient was dispatched to hematology department of Ankara University Hospital. Ultrasonographic examination of the patient revealed an intrauterine ex fetus with a biometry fitting to 16th gestational week. Pregnancy termination was decided and achieved by introduction of 100mg misoprostol. On the second day of the termination the patient applied to our clinic complaining of backache and shortness of breath. Color Doppler evaluation of the deep leg veins was normal, whereas ventilation perfusion sintigraphy and computurized tomography angiography revealed pulmonary thromboembolism in the basal segment of the right lung. Coagulation screening was completely normal. DNA analyses by polymerase chain reaction amplification revealed homozygosity for factor V Leiden gene mutation.
