Vaginal effectiveness of tibolone treatment in post menopausal women

Özet
Postmenopozal dönemdeki olgularda vaginal epitel hücre matürasyonunda tibolonun etkisinin araştırılması amacıyla bu çalışma yapılmıştır. Hastanemiz Menopoz Kliniğine başvuran, postmenopozal dönemde 50 olgu çalışma kapsamına alındı. Olgulardan 25'i çalışma grubunu, 25'i kontrol grubunu oluşturdu. Tüm olgulardan başlangıç ve altı ay sonrasında vagina yan duvarından smear alındı. Çalışma grubundaki olgulara altı ay süresince 2,5 mg/gün tibolon (Livial®; Organon) verildi. Sitolojik inceleme sonrasında karyopiknotik indeks ve maturasyon değeri hesaplandı. Ayrıca olgular vaginal yakınmaları açısından sorgulandı. Çalışma grubunda sitolojik inceleme sonrasında, matürasyon değerinde istatistiksel olarak önemli fark (p<0.017) bulundu. Karyopiknotik indekste artış saptanmasına karşın, istatistiksel olarak önemli fark (p< 0.34) saptanmadı. Kontrol grubunda her iki değerde de düşme saptandı. Olguların tibolon kullanımıyla şikayetlerinde (disparoni, vaginal kuruluk) belirgin azalma olduğu saptandı. Tibolonun, vaginal epitel matürasyonu üzerinde, klinik değerlendirme ve sitolojik inceleme sonrasında östrojenik etkisinin olduğu bulundu.
Abstract
OBJECTİVE: The purpose of this study is the investigation of vaginal effectiveness of tibolone treatment in post menopausal women STUDY DESIGN: Fifty postmenopausal women who referred to SSK Ankara Women's Health and Maternity Hospital Menopause Clinic were included in this study. Study group was composed of twenty five women, and the other twenty five women were the control group. Vaginal smears were performed from all cases by wooden spatula from the upper down to half vaginal lateral wall, at the first visit and six months later. Study group received 2.5 mg/day tibolone (Livial, Organon) for six months. KPI and MV were calculated. All cases were asked about vaginal symptoms. RESULTS: In the study group, following the cytologic investigation, a statistically significant difference (p<0.017) is found in MV. Although an increase in KPI is observed, a statistically significant difference is not obtained (p<0.34). In the control group, both values are decreased. The use of tibolon, decreased the complaints of the patients (dryness, dysparenia, etc.). CONCLUSION: Following clinic examinations and cytologic investigations, it is observed that tibolon has oestrogenic effects in vaginal epitel maturation.
