Evaluation of ultrasonographic endometrial thickness in asymptomatic postmenopausal women by hysteroscopy and dilatation and curettage

Özet

Bu çalışmada ultrasonografik olarak artmış endometrium kalınlığı tespit edilen asemptomatik postmenopozal hastalarda uterin kavitenin tanısal histeroskopi ve dilatasyon-küretaj (D&C) ile değerlendirilmesi amaçlanmıştır. En az bir yıldır menopozda olup endometrium kalınlığı 5mm ve üzerinde olan 32 hasta çalışma kapsamına alındı. Olguların endometrium kavitesi ofis histeroskopi ile değerlendirildikten sonra dilatasyon & küretaj uygulandı ve sonuçlar histopatoloji ile karşılaştırıldı. Ofis histeroskopide normal kabul edilen (atrofik endometrium) 12 olgunun (%37.5) ortalama endometrium kalınlığı 5.58±0.66mm olarak değerlendirildi. Histeroskopide patolojik olarak değerlendirilen 20 olgunun (%62.5) ise endometrium kalınlığı ortalaması 8.9±1.99mm olarak bulundu. Ofis histeroskopinin sensitivitesi %100, spesifisitesi %77.7, pozitif prediktif değeri %92 ve negatif prediktif değeri %100 olarak saptandı. Histopatolojik sonuçta normal kabul edilen (atrofik endometrium) 7 olgunun (%21.87)) ortalama endometrium kalınlığı 5.42±0.53mm idi. Anormal kabul edilen 25 olgunun (%78.13) ise ortalama endometrium kalınlığı 8.28±2.20mm olarak değerlendirildi. Anormal veya kuşkulu ultrasonografilerde diagnostik yaklaşım olarak histeroskopi-biyopsi kombinasyonu ile intrauterin patolojileri %100' e yakın bir oranda teşhis etmek mümkündür. Bu nedenle histeroskopi-biyopsi kombinasyonunun endometrial patolojilerin değerlendirilmesinde oldukça faydalı olacağı kanaatindeyiz.

Abstract

OBJECTIVE: In this study, we aimed to evaluate postmenopausal women with an increased endometrial thickness by hysteroscopy and dilatation & curretage. STUDY DESIGN: 32 postmenopausal women with an endometrial thickness of >5mm were included in the study. First hysteroscopy and then dilatation & curretage was performed. The results were compared with hystopathological findings. RESULTS: 12 women (37.5%) who were estimated as normal (atrophic endometrium) by hysteroscopy had an avarage of 5,58±0,66 mm endometrial thickness while 20 cases (62.5%) who were evaluated as pathological by hysteroscopy had an avarage of 8.9±1.99 mm endometrial thickness. Office hysteroscopy came out with a sensitivity of 100% and a specivity of 77.7%. Positive predictive vlue was 92% and negative predictive value was 100%. Hystopathologically normal (atrophic endometrium) 7 cases (21.87%) had a mean endometrial thickness of 5.42±0.53 mm. On the other hand 25 cases (78.13%) who were hystopathologically abnormal had a mean endometrial thickness of 8.28±2,20 mm. CONCLUSION: When abnormal or suspicious ultrasonographic findings are in question, it is possible to determine intrauterin pathological lesions by hysteroscopy-biopsy combination with an accuracy near to 100 %. We conclude that hysteroscopy-biopsy combination have great benefits in evaluating the endometrial lesions.
