Hyperandrogenism and nonhirsute oligomenorrheic women

Özet
Hiperandrojenik ve nonhirsut oligomenoreik kadınların durumlarını sağlıklı kontrol grubundaki kadınlarla karşılaştırmak. Ocak-Mart 1998 yılları arasında hastanemizin infertilite ve jinekoloji kliniklerine başvuran 50 hasta değerlendirildi. Kontrol grubunda 25 sağlıklı kadın (Grup 1) ve çalışma grubunda 25 oligomenoreik nonhirsut kadın vardı (Grup 2). Çalışma grubu 2 subgruba ayrıldı. Grup 2a'da mensleri 45 günden kısa süren 4 hasta, Grup 2b'de ise mensleri 45 günden uzun süren 21 hasta vardı. Tüm hastaların total testosteron, SHBG, serbest testosteron, androstenedion, DHEA-S, 17-OH progesteron ve prolaktin seviyeleri değerlendirildi. Grup 2a'nın total testosteron seviyeleri diğer gruplardan belirgin olarak yüksekti (p<0.05). Grup 2a ve Grup 2b'nin serbest testosteron ve androstenedion seviyeleri kontrol grubuna göre (Grup 1) belirgin olarak yüksekti (serbest testosteron için p<0.05 ve androstenedione için p<0.001). Çalışma ve kontrol grupları karşılaştırıldığında androstenedion'un spesifisite ve sensitivitesi belirgin olarak farklıydı (p<0.01). Hiperandrojenik ve nonhirsut oligomenoreik kadınların durumlarını açığa kavuşturmak için daha fazla sayıda hastanın değerlendirildiği ileri çalışmalara gerek vardır.
Abstract
OBJECTIVE: The aim of our study was to search hyperandrogenic states of nonhirsute oligomenorrheic women and compare them with healthy control ones. STUDY DESIGN: 50 Patients attended infertility and gynecology clinics of our hospital between Jan-Mar 1998. There were 25 healthy women in the control group (Group 1) and 25 nonhirsute oligomenorrheic patients in the study group (Group 2). We divided the study group into 2 subgroups. Group 2a had 4 patients who had menses shorter than 45 days. In Group 2b, there were 21 patients with menses longer than 45 days. Total testosterone, SHBG, free testosterone, androstenedione, DHEA-S, 17-OH progesterone and prolactin levels were evaluated. RESULTS: Group 2a had significantly higher total testosterone levels than other groups (p<0.05). Free testosterone and androstenedione levels of Group 2a and Group 2b were significantly higher than the control group (Group1) (p<0.05 for free testosterone and p<0.001 for androstenedione). Specificity and sensitivity of androstenedione was significantly different when study and control groups were compared (p<0.01). CONCLUSION: We think that further studies consisting of higher number of patients are required in order to clarify the hyperandrogenic states of nonhirsute oligomenorrheic women.
