HELLP syndrome and impact of extended period of corticosteroid administration

Özet
Uzun süreli kortikosteroid kullanımının HELLP sendromuna etkisini değerlendirmek. Haziran 1998 ile Haziran 2001 tarihleri arasında tedavi edilen 46 antenatal HELLP tanısı konan hastanın verileri incelenerek aldıkları kortikosteroid tedavisinin doz ve süresine göre 3 çalışma grubu oluşturdu. 1. gruptaki 18 hastaya serebral ödem nedeniyle günde toplam 24 mg dexamethasone 72 saat süreyle verilmişti ve sonra doz azaltılarak kesilmişti, 2. gruptaki 11 hastaya 12 saat arayla 2 defa 12 mg betamethasone verilmişti ve 3. gruptaki 17 hastaya steroid verilmemişti. Hastaların yaş, gravida, parite, gestasyonel yaş, HELLP sendromunun klası, yatış ve postpartum kan basınç değerleri, 1. ve 3. gün trombosit sayısı, AST, ALT, BUN, kreatinin değerleri, rebound fenomeni, kan transfüzyonu gereksinimleri, doğum şekilleri, hastanede kalış süreleri, mortalite ve komplikasyon oranları kaydedildi. Grupların özellikleri benzer idi. Steroid ile tedavi edilen gruplarda trombosit sayıları arasında istatistiksel olarak anlamlı yükselmeler saptandı. Rebound fenomeni oranı uzun süreli steroid kullanan grupta anlamlı olarak düşük idi. Uzun süreli steroid kullanılan HELLP tanılı hastalarda trombosit sayısı anlamlı olarak daha hızlı düzelmiş ve daha düşük oranda rebound fenomeni saptanmıştır. Bu bulgular bu tedavi şeklinin seçilmiş ağır hastalarda daha stabil bir durum oluşturmada faydalı olabileceği kanaatini uyandırmıştır.
Abstract
OBJECTIVE: To determine the impact of extended corticosteroid administration on HELLP syndrome. STUDY DESIGN: Cases of 46 women with antepartum HELLP syndrome managed between June 1998 and June 2001 were reviewed. Patients were classified on the basis of exposure to corticosteroids and the dose used. The 18 patients in group 1 received dexamethasone for cerebral edema, 24 mg daily in divided doses for 72 hours, then the dose was tapered slowly, 11 patients in group 2 received 2 doses of 12 mg betamethasone 12 hours apart and 17 patients in group 3 did not receive corticosteroids. Data collected included age, gravidity, parity, gestational age, HELLP syndrome classification, blood pressure, platelet count, AST, ALT levels, BUN, creatinine levels, on admission and on postpartum first and third days the rate of rebound phenomenon, the rate of transfusion needs, the route of delivery, the mean days of hospital stay, the mortality rate and complications. RESULTS: The characteristics of the groups were similar on admission. The steroid treated groups had significant changes over time in platelet counts on post partum day 1 and 3. The rate of rebound phenomenon was significantly lower in the extended period steroid treated groups. CONCLUSION: Patients with antenatal HELLP syndrome who received extended period corticosteroid treatment had an accelerated recovery from their platelet count and demonstrated a significantly reduced rate of rebound phenomenon which indicates that it would be more beneficial in producing a more stable state in selected cases.
