Are electronic fetal monitoring and meconium stained amniotic fluid effective on prediction of fetal outcome?

Özet

Elektronik fetal kalp atımı (FKA) monitorizasyonunun etkinliğinin daha once tanımlanmış bir sınıflamaya göre etkinliğinin araştırılması. Ayrıca mekonyum ile boyanmış amnion sıvısının perinatal sonuçlara etkisinin belirlenmesi amaçlandı. 516 gebede doğumun ilk evresinde Elektronik FKA traseleri kaydedildi. Bu traseler aynı obstetrisyen tarafından (YEE); normal, fetal distress veya fetal stress olarak gruplandı. Amnion sıvısında mekonyum varlığı da doğum sırasında belirlendi. Bu bulgular ile doğum sonuçları arasındaki ilişki araştırıldı. Kalın mekonyum boyanması olan grupta belirgin olarak daha fazla fetal asidemi oranı ve yenidoğan yoğun bakım ünitesi başvurusu oranı saptandı. Ortalama apgar skorları da kalın mekonyum olan grupta belirgin olarak düşük bulundu. Fetal asideminin tespitinde elektronik FKA monitörizasyonunun sensitivitesi %90.1 iken mekonyum boyanmasıın sensitivitesi %25.4 olarak hesaplandı. Fetal asidoz ve fetal distress'in belirlenmesinde elektronik FKA monitörizasyonu etkin ve yararlı bir metot olarak değerlendirilmiştir. Bu test fetal asidoz veya stress olmayan fetusların belirlenmesinde daha etkili gibi görünmektedir. Amnion sıvısının mekonyum ile boyanması fetal distress'in belirlenmesinde etkili bir yöntem olarak görülmedi.

Abstract

OBJECTIVE: To determine the effectiveness of electronic fetal heart rate (FHR) monitoring according to a previously defined classification. We also aimed to determine the effects of meconium-stained amniotic fluid on perinatal outcome. STUDY DESIGN: Electronic FHR tracings of 516 pregnant women were recorded during the first phase of labor. These tracings were evaluated and classified as being normal, as fetal distress, or as fetal stress by a single obstetrician (YEE). Presence of meconium staining of amniotic fluid and fetal acidosis were also determined during delivery. Association between these findings and perinatal outcome was evaluated. RESULTS: Significantly higher ratios of fetal acidemia and neonatal care unit admissions were found in groups with thick meconium staining. Mean apgar scores were also significantly lower in this group. Sensitivity of electronic FHR monitoring for determination of fetal acidemia was 90.1% whereas sensitivity of meconium staining was 25.4%. CONCLUSION: Electronic FHR monitoring tracing is an effective and useful method for determination of fetal acidosis and distress. It seems that this test is more useful for determination of fetus without acidosis or stress. Meconium staining of amnion seems not an effective test for fetal distress.
