Measurement of IGFBP-1 in cervicovaginal secretions in the diagnosis of premature rupture of membranes

Özet
Erken membran rüptürü (EMR) gebeliklerin % 5-8'inde görülen ve önemli oranda perinatal morbidite ve mortaliteye neden olan bir problemdir. EMR tanısının doğru konması ve uygun yönetim maternal ve fetal morbiditeyi azaltacaktır. Bu amaçla birçok test geliştirilmiştir; Ntrazin testi, Fern testi, Bromtimol blue testi, servikovajinal sıvıda fetal fibronektin ölçümleri. Bu testler kan, semen, enfeksiyon gibi faktörlerden etkilendiklerinden spesifite ve sensitiviteleri azalmaktadır. Amniotik sıvıda diğer vücut sıvılarından 100 kat daha yüksek oranda bulunan IGFBP-1'in servikovajinal sıvıda immünokromotografik yöntemle ölçümü esasına dayanarak yapılan PROM (Promature Rupture of Membranes) testinde spesifite ve sensitivite diğer testlerden daha yüksek saptandı. Bu çalışmaya 20-37 gebelik haftası arasındaki intakt membranı olan 30 asemptomatik kadın, klinik olarak doğrulanmış membran rüptürü olan 20 kadın ve EMR şüphesi olan 10 kadın olmak üzere toplam 60 kadın dahil edildi. Servikovajinal sekresyonlarda IGFBP-1 immünokromotografik yöntemle ölçüldü. PROM testin spesifitesi %100, sensivitesi %96.6, pozitif prediktif değeri %100, negatif prediktif değeri %96.6 olarak bulundu. Sonuç olarak PROM testi oldukça hızlı ve güvenilir bir testtir, spesifite ve sensivitesi yüksektir. Prematür membran rüptürü tanısında kullanılabilecek uygun ve pratik bir yöntem olduğu vurgulanmıştır.
Abstract
OBJECTIVE: In this study we aimed to prove that IGFBP-1 scanning in cervicovaginal secretions is a fast, reliable and appropriate method for the diagnosis of premature rupture of membranes. STUDY DESIGN: Thirty asymptomatic pregnant women with intact membranes, twenty women with clinically proven membrane rupture and 10 pregnant women with a suspect of PROM were included in this study. IGFBP-1 levels were determined immunochromatographically in cervicovaginal secretions. RESULTS: Spesificity, sensitivity, positive predictivity and negative predictivity of PROM test were 100%, 96.6%, 100% and 96.9%, respectively.CONCLUSION: As a result, PROM test is a fast and safe test with high specifity and sensitivity.It is concluded that this test can be used in diagnosis of PROM in daily practice.
