Reprodüktif çağın en sık rastlanan over tümörlerinden olan matür kistik teratomların malign dönüşümüne sıklıkla postmenopozal kadınlarda rastlanır. En sık görülen histolojik tip yassı hücreli karsinomdur. Bu çalışmada Ocak 1993-Ağustos 2000 tarihleri arasında kliniğimizde tedavi gören dört matür kistik teratom zemininde gelişmiş yassı hücreli kanser olgusu sunuldu. Hariçte opere edilip kliniğimize refere edilen bir olgu dışındaki tüm olgulara evrelendirme yapıldı. Iki olgu evre IA bulunurken diğer bir olgu evre IIIC olarak değerlendirildi. Bir olguya adjuvan tedavi olarak radyoterapi uygulanırken, diğer üç olguya platin ağırlıklı kemoterapi verildi. Evre IA olan bir olgu ve IIIC olan bir olgu kemoterapinin dördüncü kürünü alırken kaybedildi. Bu tümörlerde prognozun overin primer yassı hücreli karsinomlarına göre daha kötü olduğu bilinmektedir, bu nedenle özellikle stage IB ve üzeri olgularda adjuvan tedavi önerilmektedir.
Abstract
Malign transformation of mature cystic teratoma, which is one of the most frequent tumors of reproductive age, is generally seen in postmenopausal women. The most prevalent histological type is squamous cell carcinoma. In this study malignant transformation of four mature cystic teratoma cases, treated between January 1993 and August 2000 in our department, are reported. All cases staged except one case that underwent surgery in her local hospital. Two cases were stage IA and the other one case was stage IIIC. Three cases received platine-based chemotherapy, while one case underwent radiotherapy as adjuvant therapy. The stage IIIC case and one of the stage IA cases died in the fourth course of chemotherapy. Prognosis of this tumor is worse than primary squamous cell carcinomas of the ovary; so that adjuvant therapy is suggested especially for the patients stage IB and higher stages.
