Cystic leukomalacia in a term infant whose co-twin died in utero

Özet

38 haftalık gebelik sonunda normal spontan vajinal yolla 2400 g ikiz eşi olarak doğan kız bebekte postnatal altıncı günde kistik lökomalazi saptanmıştır. Doğumda eşinin ölü olduğu ve plasentanın tek olduğu kaydedilmiştir. Monokoryonik gebeliklerdeki nörolojik gelişim bozukluklarından vasküler anastomozlar ve ikizden ikize transfüzyon sorumlu tutulmaktadır. İkiz eşinin in-utero ölümüne bağlı vasküler dirençte düşme, yaşayan eşte, hipotansiyon ve kistik lökomalaziye neden olabilmektedir.

Abstract

A female twin who was born by spontaneous vaginal delivery at 38 weeks of gestation. The baby's birth weight was 2.400 g, and her Apgar scores at 1 and 5 minutes were 7 and 9, respectively. Intrauterine death of the other twin and monochorionic placentation were noted in the prenatal history. Multicystic encephalomalacia was diagnosed by cranial ultrasonography on postnatal day 6 without any evidence other than death of co-twin in utero. Monochorionic twinning with vascular anastomoses, TTTS, and ischemic damage led to the cystic encephalomalacia in this term newborn. In the case of death of one twin, the other should have a cranial ultrasound examination after the delivery.
