Analysis of 47145 deliveries in a tertiary center: An epidemiological view

Perinatal komplikasyonların insidansı değerlendirilmiştir. 01. Ocak.2000 ile 01.Ocak.2002 arasında meydana gelen 47145 doğum olgusu bu retrospektif çalışmada incelenmiştir. İncelenen popülasyonda kadınların %2.7'si 18 yaş altı, %6.4'ü 35 yaş üzeri; %46.8'i nullipar ve %0.6'sı grand multipar idi. Dekolman plasenta %0.3 olguda tespit edildi. Zarların preterm prematür yırtılması %1.2 vakada ve erken yırtılması %19.1 vakada tespit edildi. Doğan fetüslerden % 5.6'sı prematürite, %9.1'i postmatürite ve %7.2'si intrauterin gelişme geriliği gösteriyordu. Eylem indüksiyonu %8.8 olguda oksitosin ve %2.7 olguda misoprostol ile yapıldı. Maternal anemi %11.4 oranında ve trombositopeni (<100000/mm3) %1.1 olguda tespit edildi. Fetüslerden %96.5'i verteks ve % 3.5'i non-verteks prezantasyon ile doğdu. Uterin rüptür % 0.008 olguda gelişti. Vakaların %77.3'ü spontan vajinal yol doğurdu; %74.2 olguda epizyotomi açıldı. Sezaryen hızı % 22.7 idi ve %90.3 olguda spinal anestezi ile gerçekleştirildi. Üçüncü ve dördüncü derecede yırtıklar %0.6 olguda meydana geldi. Uterin atoni %2.2 olguda tespit edildi; 19 bilateral hipogastrik ligasyonu ve 8 histerektomi yapıldı. Doğum ağırlıklarının incelenmesi; fetüslerin %1'inin çok düşük doğum ağırlıklı, %7.2'sinin düşük doğum ağırlıklı, %4.8'inin makrozomik ve % 0.3'ünün ileri derecede makrozomik olduğunu ortaya koydu. Omuz distosisi %0.3 olguda ve klavikula kırığı %0.1 olguda meydana geldi. Puerperal komplikasyonların incelenmesi %0.23 epizyotomi açılması, %0.1 sezaryen sonrası insizyon açılması ve % 0.1 plasenta retansiyonu olduğunu ortaya koydu. Obstetrik yöntemlerin geliştirilmesi, insidans analizlerine ve ilgili popülasyondaki komplikasyonlara predispozan faktörlerin incelenmesine dayandırılmalıdır.
Abstract
OBJECTIVE: To evaluate the incidence of perinatal complication. STUDY DESIGN: Data of 47145 deliveries between 01.January.2000 and 01.January.2002 are presented in this retrospective study. RESULTS: Among the analysed population 2.7% were 18 years old or under and 6.4% were 35 years old or over; 46.8% women were nulliparous and 0.6% were grand multiparous. Abruption of placenta was diagnosed in 0.3% among patients. Preterm premature rupture of the membranes occured in 1.2% and premature rupture of the membranes in 19.1%. Among the fetuses 5.6% were premature, 9.1% postmature and 7.2% intrauterine growth restricted. Induction of labor was performed by oxytocin in 8.8% and misoprostol in 2.7%. Maternal anemia (Hb<10g/dl) was detected in 11.4% and thrombocytopenia (<100000/mm3) in 1.1% women. Among the fetuses 96.5% were vertex and 3.5% were non-vertex presentation. Uterine rupture occured in 0.008% cases. Spontaneus vaginal delivery was achieved in 77.3%; episiotomy was performed in 74.2%. The cesarean rate was 22.7%; 90.3% of the patients were given spinal anesthesia. Third and fourth degree perineal tears occured in 0.6% of the cases. Uterine atony was detected 2.2%, 19 bilateral hypogastric artery ligation and 8 hysterectomies were performed. Analysis of the birth weight revealed that 1% were very low birth weight, 7.2% low birth weight , 4.8% macrosomic and 0.3% extreme macrosomic (³4500g) fetuses. In 0.3% cases shoulder dystocia and in 0.1% cases clavicular fracture occured. Analysis of the puerperal complication demonstrate 0.23% episiotomy sutures break-down, 0.1% post-cesarean sutures break-down and 0.1% retention of the placenta. CONCLUSION: Steps to modernize obstetrics should be based on the analysis of the incidence and predisposing factors of complications in the target population.
