Misoprostol compared with methylergometrine for prevention of postpartum hemorrhage: A randomized controlled trial

Postpartum hemorajinin önlenmesinde oral misoprostol ve intramüsküler metilergometrin uygulamasının karşılaştırılması. Randomize kontrollü çalışma düzeni içinde normal gebeliği olan 100 kadın ya 600 mg oral misoprostol ya da standard metilergometrin tedavisi almak üzere iki gruba randomize edildi. Postpartum kanama insidansı, ilaçların yan etki insidansı ve ciddiyeti, ek oksitosik ilaç ya da kan transfüzyonu ihtiyacı, ortalama hemoglobin düzeyi, kan basıncı, obstetrik özellikler kaydedildi. Çalışmayı 100 hasta da tamamladı (50 hastaya metilergometrin, 50 hastaya 600 mg misoprostol verildi). Postpartum kanama misoprostol grubunda %6, metilergometrin grubunda %4 oranında tespit edilidi (P=0.57). Ek oksitosik tedavi ihtiyacı sırasıyla %10 ve %6 idi. Misoprostol grubunda %4 oranında kan transfüzyonu yapılırken, diğer grupta kan transfüzyonu yapılmadı. Iki grup arasında doğum öncesi (14,26±1.43 versus 13,78±1,37) (P=0.95) ve sonrası hemoglobin değerleri (11,53±1.33 versus 12,08±1,28) (P=0.78) arasında farklılık saptanmadı. Yan etki insidansı ve ciddiyeti her iki grupta benzerdi. Bu çalışma postpartum kanamanın önlenmesinde metilergometrin ve misoprostolün benzer etkinlikte olduğunu göstermektedir (P=0.57).
Abstract
OBJECTIVE: To compare the efficacy and side effects of oral misoprostol with intramuscular methylergometrine administration for the prevention of postpartum hemorrhage. STUDY DESIGN: This study was designed as a randomized controlled clinical trial of 100 women with apparently normal pregnancies. One hundred women were randomized to oral 600 mg misoprostol or to standard oxytocic regimen with methylergometrine. Main outcome measures were the incidence of postpartum bleeding, incidence and severity of side effects, need for further oxytocic drugs or blood transfusion, mean hemoglobin level and blood pressure, obstetrical outcomes and complications. RESULTS: One hundred women completed the study (50 received methylergometrine and 50 received 600mg misoprostol). Postpartum bleeding occurred in %6 of misoprostol group and in %4 of methylergometrine group (P= 0.57). The need for further oxytocic therapy was %10 and %6 respectively. Only %4 of the misoprostol group had necessity for blood transfusion while none of the other group had transfusion. There were no significant differences between two groups in prepartum (14,26±1.43 versus 13,78±1,37) (P=0.95) and postpartum hemoglobin values (11,53±1.33 versus 12,08±1,28) (P=0.78). Ratio and severity of side effects were similar in both groups. CONCLUSION: This study revealed that the prevention of postpartum bleeding by methylergometrine and misoprostol seems similar (P=0.57).
