Ovarian secretion of steroids in postmenopausal women with cancer of the endometrium

Özet
Bu çalışmanın amacı postmenopozal endometrium kanseri olan ancak bilinen risk faktörleri taşımayan hastalarda overin steroid hormon sekresyon fonksiyonunu araştırmak ve bu fonksiyonun endometrium kanseri etyopatogenezinde rolu olup olmadığına açıklık getirmektir.Endometrium kanseri tanısıyla Total Abdominal Histerektomi ve Bilateral Salpingooferektomi uygulanan 35 hasta bu çalışmanın materyalini oluşturmuştur.Kontrol grubunu ise postmenopozal dönemde olup atrofik endometriuma sahip 25 olgu oluşturmuştur. Bu hastalardan ovarian venden ve simultane olarak periferik venden Estradiol(E2), Androstenedion (A) ve serbest Testosteron (f- Tes) çalışılmıştır. Kanserli olguların periferik ve ovarian venöz kandaki serum A, E2, f-Tes düzeyleri karşılaştırıldı.Ovarian venöz kan A, E2, f-Tes düzeyleri perifere oranla belirgin olarak yüksekti. İstatistiksel olarak sadece E2 ve f-Tes için anlamlı periferik ven ile ovarian fark saptandı(p 0.05). Kanserli grupta ovarian venöz serum E2, A, f-Tes düzeyleri kontrol grubuna oranla belirgin olarak yüksek bulundu.Ancak istatistiksel olarak anlamlı fark bulunmadı (p>0.05). Kanserli ve kontrol grubunun periferik venöz serum E2, A, f-Tes düzeyleri karşılaştırıldı. Kanserli E2 belirgin yüksek A çok az yüksek ve f-Tes çok az düşük bulundu. Ancak istatistiksel olarak anlamlı fark bulunmadı(p>0.05). Yaptığımız bu çalışmada postmenopozal endometrİum kanserli hastaların ovarian venöz serumunda E2, A, f-Tes düzeyleri kontrol grubuna oranla istatistiksel olarak anlamlı olmasa da belirgin olarak yüksek bulduk. Biz postmenopozal endometrium kanserli hastaların overlerinde steroid salgılanmasındaki bu artışın endometrium kanserinin patogenezinde rol oynayabileceğini düşünüyoruz.
Abstract
OBJECTIVE: To investigate the functions of ovarian steroid hormone secretion in postmenopausal women with cancer of the endometrium but without the known risk factors and whether it played a role in etiopathogenesis of endometrial cancer. STUDY DESIGN: This study included 35 patients who were diagnosed with endometrial cancer and who underwent total hysterectomy and bilateral salpingo-oophorectomy and 25 controls with atrophied endometrium. Levels of estradiol, androstenedione and free testoste rone were investigated in blood samples simultaneously obtained from ovarian and peripheral veins. RESULTS: Ovarian serum levels of androstenedione, estradiol and free testosterone were considerably higher than peripheral serum levels of androstenedione, estradiol, and free testosterone in patients with cancer. There was a significant difference only in estradiol and free testosterone levels between peripheral and ovarian veins (p<0.05). Ovarian serum levels of androstenedion, estradiol and free testosterone were considerably higher than peripheral serum levels of androstenedione, estradiol, and free testosterone in controls, but there was no significant difference between the hormone levels (p>0.05). Ovarian venous blood levels of androstenedione, estradiol and free testosterone were considerably high in patients with cancer compared to controls with no significant difference (p>0.05). Peripheral estradiol levels were considerably high, androstenedione levels quite high and free testosterone levels slightly low in patients with cancer compared to controls, with no significant difference (p>0.05). CONCLUSION: Ovarian venous blood levels of androstenedione, estradiol and free testosterone were considerably higher in postmenopausal women with endometrial cancer than in controls, though not statistically significant. We conclude that increased secretion of steroids by ovaries of patients with postmenopausal endometrial cancer may play a role in the pathogenesis of endometrial cancer.
