Preterm twin versus singleton deliveries

Doğumda gestasyonel yaş, doğum şekli, doğum ağırlıkları ve Apgar skorları açısından preterm ikiz ve tekiz doğumları karşılaştırarak, ikiz preterm doğumlarda perinatal sonuçları değerlendirmektir. 48 ikiz ve 61 tekiz gebelik retrospektif incelendi. Fetal ve maternal komplikasyonu olan gebelikler çalışmaya dahil edilmedi. Hastaların maternal yaş, gravida, parite, doğumda gestasyonel yaş, doğum şekli, doğum kiloları ve 1' ve 5' Apgar skorları incelendi. Yukarıdaki parametreler açısından iki grup karşılaştırıldı. Maternal yaşlar açısından iki grup arasında anlamlı fark yok iken gravida=>2 ikiz gebeliklerde, nulliparite ise tekiz gebeliklerde anlamlı farkla yüksek bulundu (p 0.05). Preterm ikiz doğumlarda yüksek sezaryan oranları dikkat çekicidir. İkiz preterm doğumlarda perinatal sonuç, yenidoğanların doğum kiloları ve Apgar skorları açısından tekizlerden farklı değildir.

Abstract

OBJECTIVE: To evaluate perinatal outcome in twin preterm deliveries by comparing them with singleton preterm deliveries in means of gestational age at birth, delivery mode, birth weight and Apgar scores. STUDY DESIGN: Records of 48 twin and 61 singleton preterm deliveries were evaluated retrospectively. Pregnancies with maternal and fetal complications were excluded. Maternal ages, gravidity, parity, gestational age at birth, delivery modes, birth weights, 1' and 5' Apgar scores were analyzed. The two groups were compared for the above parameters. RESULTS: Although, no significant difference was found betwen the two groups when compared for maternal ages, significantly higher rate of nulliparity was found in singletons and mothers of twins tended to be of higher gravidity (p<0.05). A significantly higher gestational age at birth was found in twin preterm deliveries (p<0.05). Cesarean section was significantly more common in twins (p<0.05). No significant difference was found in Apgar scores and birth weights of the neonates (p>0.05). CONCLUSION: The high cesarean rate of twins is challenging. In twin preterm deliveries, perinatal outcome, in terms of Apgar scores and birthweights of the neonates, is not different from that of singletons.
