Clinical experience with uterine sarcomas: Study of 17 cases

Özet
Bu çalışmanın amacı, uterin sarkoma'ların: Klinik ve histolojik özelliklerini, hastalığın insidansını, survival'ını ve tedavi modalitelerini, Türkiyedeki referans hastanelerinden bir tanesi olan hastanemizde, araştırmaktır. Zekai Tahir Burak Kadın Sağlığı, Eğitim ve Araştırma hastanesinde, Eylül 1993 ve Mart 2000 tarihleri arasında, uterin sarcoma tanısı ile tedavi edilen 17 hastanın, tıbbi kayıtları ve histolojik preparatları incelenmiştir. Hastaların yaşları, tanı anındaki semptomları ile operatif ve patolojik bulguları değerlendirilmiştir. Bu çalışmada Rabdomyosarkoma(RMS), Malign mixed müllerian tümor (MMMT), endometrial stromal sarcoma (ESS) ve leiomyosarkoma (LMS) insidansları sıra ile %11.6, %17.4, %24, %47 olarak tespit edildi. ESS vakalarının %50'si benign nedenlerle opere edildikten sonra tanı aldı. Endometrial sarkomalı vakaların tümü düşük grade'li iken, MMMT vakalarının tümü homolog farklı tümor tiplerinden oluşmuştu. 9 hasta (%53) tanı anında evre 1'de idi. Hastalardaki başlıca s emptom, abdominal kitle idi (%44). Diğer semptomlar ise anormal vajinal kanama (%28), postmenopozal kanama (%17), ve abdominal ağrı (%11) idi. 5 vakada endometrial biopsi ile malignite saptandı. Tüm hastalarımız için saptadığımız genel 5 yıllık yaşam oranı % 29.4 idi. Bu oran LMS için %12.5, MMMT için % 34, RMS icin %0 iken düşük grade'li ESS için 5 yıllık yaşam %100 idi. Hastalarımızdan 7 tanesi (%47) postoperative radyoterapi, 6 tanesi postoperative kemoterapi (%35) ile 3 tanesi de (%18) her iki metodla tedavi edildiler. Yalnız 1 hasta (%6) tek başına cerrahi ile tedavi edildi. Uterin sarkoma'lar hızlı ilerleyen ve oldukça kötü prognozlu tümörlerdendir. Ana prognostic faktörler: tümörün histopatolojik tipi ve evresidir. Rabdomyosarkomalı hastalar en kötü prognoza sahip iken, düşük grade'li endometrial stromal sarcoma diğer histolojik tiplerine nazaran daha iyi prognoza sahiptir. Uterin sarkoma' larda standart Tedavi cerrahi'dir. Adjuvant radyoterapi ve/veya sistemik kemoterapi halen tartışmalıdır
Abstract
OBJECTIVE: The aim of this study was to investigate the clinical and histological features of uterine sarcomas and to evaluate treatment modalities, survival and the incidence in one of the referral hospitals in Turkey. STUDY DESIGN: Medical records and histology slides of 17 patients with uterine sarcoma who were treated, between September 93 and March 2000, at the Zekai Tahir Burak woman Health, Education and Research Hospital were reviewed. Patient's ages, admitting symptoms, and operative and pathologic findings were analysed. RESULTS: The incidence of rhabdomyosarcoma (RMS), malignant mixed Mullerian tumour (MMMT), endometrial stromal sarcoma (ESS), and leiomyosarcoma (LMS) in this study was 11.6%, 17.4%, 24%, 47% respectively. 50% of the ESS was diagnosed after being operated for a benign reason. All of the endometrial stromal sarcomas were low grade, whereas all of the MMMT were homologous variants of the tumour. 9 (53%) patients were stage I at the time of diagnosis. The symptomatology consisted mainly of abdominal mass (44%). The other symptoms were abnormal vaginal bleeding (28%), postmenopausal bleeding (17%), and abdominal pain (11%). For 5 cases endometrial biopsy established the malignancy. Our overall five years survival was 29.4%: 12.5% for the LMS, 34% for MMMT, 0% for RMS whereas the five year survival was 100% for low grade ESS. 7 (47%) patients had postoperative radiotherapy, 6 (35%) were treated with postoperative chemotherapy, whereas 3 (18%) patients received both and 1 (6%) was treated with surgery alone. CONCLUSION: Uterine sarcomas are rapidly progressing tumours with a rather poor prognosis. The main prognostic factor is the histopathologic type and the stage of the tumour. Patients with rhabdomyosarcoma has the worst prognosis while, patients with low- grade endometrial stromal sarcoma had a more favourable prognosis than those with other histological types. The standard therapy is surgery. Adjuvant radiotherapy and/or systemic chemotherapy are still controversial.
