Trabekülektomi sonrası gelişen koroid dekolmanları: İlişkili faktörler ve ameliyat başarısına etkisi

Özet
Amaç: Trabekülektomi sonrası koroid dekolmanı(KD) gelişimi üzerine etkili olabilecek faktörleri ve KD’nin trabekülektominin uzun dönemde göz içi basıncı(GİB) kontrolü sağlamadaki başarısına etkisini incelemek. Gereç ve Yöntem: Glokom birimimizde Nisan 1993-Eylül 2003 arasında trabekülektomi uygulanmış tüm hastaların dosyaları retrospektif olarak incelendi. Bunlardan en az 1 yıl düzenli takip edilmiş 198 hastanın 253 gözü çalışmaya alındı. Bunlardan KD gelişen 28 hastanın 28 gözü grup 1, geri kalan 170 hastanın 225 gözü grup 2 olarak alındı. Bulgular: Grup 1’de preoperatif görme keskinliği kontrol grubuna göre anlamlı olarak daha düşük, cup/disk oranı daha yüksek ve psödoeksfolyatif glokom sıklığı daha fazla bulundu. Ameliyat öncesi bulgulardan ön kamara (ÖK) daralması, hipotoni, hipotonik makulopati, hifema varlığı ve ÖK’de fibrin reaksiyonu ile KD arasında anlamlı ilişki saptandı. Grup 1’de KD gelişiminin görme keskinliğinde kalıcı düşüşe neden olmadığı tespit edildi. Ameliyat öncesi 1. gün, 6. ay ve 1. yılda KD gelişen grupta kontrol grubuna göre GİB’nın istatistiksel olarak anlamlı şekilde daha düşük olduğu saptandı. GİB’nın 18 mmHg’nin altında olma başarısı açısından 2 grup arasında anlamlı fark bulunmadı. Kullanılan glokom ilacı sayısı açısından 6. ay ve 1. yılda gruplar arasında anlamlı fark yokken 2. yılda grup 1’de daha yüksek bulundu. Sonuç: Trabekülektomi sonrasında gelişen KD’nin görme keskinliği ile uzun dönemde trabekülektomi başarısı ve GİB kontrolü üzerine kalıcı etkisi bulunmamaktadır.
Abstract
Purpose: To evaluate the factors affecting the development of choroidal detachment (CD) after trabeculectomy and the effect of CD on the long term success of trabeculectomy in controlling intraocular pressure (IOP). Materials and Methods: Patients who had been applied trabeculectomy from April 1993 to September 2003 were reviewed retrospectively. Two hundred and fifty three eyes of 198 subjects who had at least 1 year follow up were included in the study. Twentyeight eyes of 28 subjects in which CD developed were named as Group 1 and the rest 225 eyes of 170 subjects were named as Group 2. Results: In group 1, preoperative visual acuity was considerably less, cup/disc ratio was increased and the frequency of pseudoexfoliative glaucoma was high with respect to the control group. There were significant relations between the development of CD and preoperative findings such as narrowing of anterior chamber, hipotony, maculopathy of hipotony, hyphaema and fibrin reaction in the anterior chamber. In group 1, it is determined that CD is not the cause of permanent decrease in visual acuity. In the preoperative 1st day, 6th month and 1st year follow up, the decrease in IOP in CD group was statistically significant when compared to the control group.There was no significant difference between both groups according to the success of decreasing IOP under 18 mmHg.There was also no significant difference between the two groups when the number of glaucoma medications used in 6th month and 1st year are compared but in the 2nd year folllow up period it was higher in group 1. Conclusion: Choroidal detachment which may occur after trabeculectomy has no permanent effect on visual acuity, long term success of trabeculectomy and IOP control.
