Fakik ve Psödofakik Primer Açık Açılı Glokomlu Hastalarda Selektif Lazer Trabeküloplasti'nin Etkinlik ve Güvenilirliği

Efficacy and Safety of Selective Laser Trabeculoplasty, in Phakic and Pseudophakic, Primary Open Angle Glaucoma Patients

Özet

Amaç: Fakik ve Psödofakik hastalarda, Selektif Lazer Trabeküloplasti (SLT)'nin etkinlik ve güvenilirliğini incelemektir.Gereç ve Yöntem: Bu retrospektif çalışmada, Primer Açık Açılı Glokom (PAAG) tanısı olan; Fakik (Grup I, n=16) ve Psödofakik (Grup II, n=19) hasta gruplarında, medikal tedaviye rağmen, göz içi basıncı (GİB) >21 mmHg olması nedeni ile, 'ek tedavi' olarak 1 seans SLT uygulandı. Hastalar SLT'den 1 saat sonra, geçici GİB artışı ve inflamatuar reaksiyon açısından kontrol edildi. Hastaların SLT tedavisi öncesi ve sonrası 1. hafta, 1. ay, 3. ay, 6. ay, 12. ay GİB bulguları değerlendirildi. Başarı; GİB'te, >3 mmHg veya >%20 azalma olarak tanımlandı.Bulgular: Grup I ve Grup II'de; tüm kontrollerde SLT, GİB'i anlamlı olarak düşürdü. Gruplar arasında, SLT tedavisi öncesi ve sonrası bütün takiplerdeki GİB değerleri ve yan etki açısından anlamlı fark yoktu. Çalışmamızda SLT'nin GİB'i düşürücü etkisi; 1. hafta ve 3. ayda Grup I'de, Grup II'den anlamlı olarak daha fazla iken 1. ay, 6. ay ve 12. ay kontrollerinde gruplar arasında anlamlı fark gözlenmedi. SLT sonrası 12.ay ölçümlerindeki; (Grup I ve Grup II sırası ile) GİB'de ortalama düşüş, 4.0±1.79 mmHg (p=0.001) ve 5.11±2.38 mmHg (p=0.001); başarı oranı, %75 ve %79 idi.Sonuç: Fakik ve psödofakik hastalarda SLT tedavisi, GİB'i düşürmede benzer derecede etkin ve güvenilirdir

Abstract

Purpose: To investigate the efficacy and safety of selective laser trabeculoplasty (SLT) in phakic and pseudophakic, Primary Open Angle Glaucoma (POAG) patients.Materials and Methods: Phakic Group (Group I, n=16) and Pseudophakic Group (Group II, n=19) patients with POAG and intra ocular pressure (IOP) >21 mmHg, despite the medical treatment, had SLT treatment in this retrospective study. One hour after SLT, temporary IOP elevation and inflammatory reaction were controlled. Also, IOP before SLT and after SLT at 1 week, 1 month, 3 months, 6 months, and 12 months were evaluated. 'Success' was defined as; IOP decrease >3 mmHg or >% 20.Results: Following the SLT, at each time interval, there was a significant IOP decrease in both Groups. There was no significant difference, between Group I and II, for IOP before SLT, IOP after SLT and side effects, at each time interval. IOP lowering effect of SLT was more significant in Group I than Group II, at 1.week and 3.months controls, although there were no significant difference at 1 month, 6 months and 12 months controls. Twelve months after SLT, mean IOP decrease were (Group I and II, in turn) 4.0±1.79 mmHg (p=0.001) and 5.11±2.38 mmHg (p=0.001). Success rates were 75% and 79%. Conclusion: In pseudophakic and phakic patients, SLT has similar IOP decreasing effect and safety
