İntravitreal Triamsinolon Enjeksiyonu Sonrası Pupiller Blok Glokomu

Özet
İntravitreal triamsinolon asetonid (İVTA) enjeksiyonu sonrası göz içi basınç (GİB) artışı karşılaşılan önemli problemlerden biridir. 56 yaşındaki diyabetik hasta sağ gözünde İVTA enjeksiyonundan bir gün sonra başlayan görmede azalma şikayeti ile başvurdu. Sağ göz muayenesinde; görme keskinliği 1 metreden parmak sayma, GİB 67 mm Hg, kornea ödemli ve iris öne doğru bombe idi. Ön kamara (ÖK) santralde sığ periferde ise silik olduğu gözlendi. Pupiller blok glokomu tanısı kondu. Aköz drenajını sağlamak için yapılan Nd: YAG lazer iridotomi den 60 dakika sonra sağ göz GİB 16 mmHg'ye düştü ve ÖK derinliğinin arttığı görüldü. Olgumuz İVTA enjeksiyonu sonrasında erken dönemde pupiller bloğa bağlı GİB'da yükselme olabileceğini göstermesi açısından önemlidir
Abstract
Traumatic globe rupture usually occurs at its weakest point. Completely healed surgical wound of extracapsular cataract extraction does not have the same strength as the intact globe even decades after the surgery. In this study, a rare case of traumatic globe rupture without conjunctival injury following six years of cataract surgery is presented. This study emphasize that the key point of the surgical technique is "not to open the conjunctiva until the visible scleral wound is closed". By using this approach normalized intra ocular pressure in this type of injury may be preserved and also more serious complications such as choroidal hemorrhage and detachment may be prevented during the surgical management
