Propofol ağrısını önlemede çeşitli ilaç ve yöntemlerin karşılaştırılması

Özet
Hızlı ve güvenli bilinç kaybı oluşturan intravenöz bir ajan olan propofolün yüksek injeksiyon sonrası ağrı insidansı vardır. Bu çalışmada farklı tekniklerin etkinliğini karşılaştırmayı ve en etkin olanı bulmayı amaçladık. Çalışmaya elektif çeşitli girişimler geçirecek 160 ASAI-II olgu dahil edildi. Çalışma rastgele ve çift-kör olarak planlandı. Çalışmaya dahil olan olgular 20 şerli olarak sekiz gruba rastgele ayrıldı. İntravenöz kanül el sırtına yerleştirildi. Önkola turnike yerleştirildikten sonra, 2ml % 0.9 NaCI (Grup P), 4$\circ$ C propofol (Group Isı), 0.5$\mu$g.kg-1 remifentanil (GrupR), 1mg alfentanil (GrupA), 50mg tramadol (GrupT), 4mg ondansetron (GrupO), 40mg lidokain (GrupL), 10mg metoklopramid (GrupM) verildi. Olgulara kollarında ağrı olup olmadığı soruldu ve gözlendi ve cevaplar kaydedildi. Veriler Student t ve Ki-kare testleri ile değerlendirildi ve p<0.05 anlamlı kabul edildi. Tüm gruplarda ağrı skoru GrupP ile karşılaştırıldığında düşük bulundu. Grup L, R, A ve M nın ağrı skorları, Group P, Isı, O ve T den düşüktü ve istatistiksel olarak anlamlı idi. Sonuç olarak; Grup L de 16 olgu, Grup R de 15 olgu ve Grup A da 12 olguda hiç ağrı gözlenmedi. Spontan ağrı Grup R ve A da görülmezken Grup L de sadece bir olguda vardı. Bu sonuçlara dayanarak lidokainin yanı sıra remifentanil ve alfentanil propofol injeksiyon ağrısını önlemede ilk seçilecek ilaçlar olarak kabul edilebileceği kanısındayız.
Abstract
Propofol is an intravenous agent which rapidly and reliably causes loss of consciousness also it has a high incidence of pain with injection. In this study we aimed to compare the efficacy of different tecniques and find out the most effective one. The study was conducted on 160 ASA l-ll consenting patients who were to undergo various elective operations. The study design was randomized and double-blind. The patients included in the study were randomly assigned to eight groups of 20 patients each. An intravenous cannula was inserted in the dorsum of the hand. After tourniquet application to the forearm, % 0.9 NaCI 2ml (Group P), 4$\circ$ C propofol (Group Isı), remifentanil 0.5$\mu$g.kg-1 (GroupR), alfentanil 1mg (GroupA), tramadol 50mg (GroupT), ondansetron 4mg GroupO), lidocaine 40mg (GroupL), metochlopramid 10mg (GroupM) was administrated: The patients were observed and asked if they had pain in the arm and the response was assessed. All the data were analized by using Student t and Ki-square tests and p<0.05 was accepted significant. All the groups pain scores were less when compared with Group P. Group L, R, A and M's pain scores were less than Group P, Isi, O and T and the differences were significant. As a conclusion; 16 patients in Group L, 15 patients in Group R and 12 patients in Group A reported no pain. No spontan pain was seen in Group R ve A but just one patient in Group L. According these results, besides lidocaine we can accept remifentanil and alfentanil as first choice drugs in preventing propofol injection pain.
