Özet

Fetusun, plesantanın ve eklerinin karın ve uterus duvarlarına yapılan insizyonlardan doğurtulması anlamına gelen cesarean section'un alt transvers kesisinin iki farklı teknikle kapatılmasını klasik teknikle karşılaştırdık. Cesarean section obstetride en sık kullanılan operasyon şeklidir. 1970'lerden itibaren artan ve günümüzde %30'lara varan uygulanma sıklığı bunu göstermektedi. Bu amaçla Ocak 1998'den itiberen kliniğimizde cesarean section operasyonlarında uterus alt segment kesisini yeni teknikle kapattık. 82 gebe olgu, çalışma grubunu oluştururken, 40 gebe olgu kontrol grubunu oluşturdu. Kontrol grubunda uterus alt segment kesisinde myometrium iki kat sütürle kapatılmıştır. Her iki gruptaki hastalar operasyon süresi, komplikasyonlar, yara yerinin ultrasonografik incelenmesi ve kullanılan cerrahi materyal miktarı açısından karşılaştırılmıştır. Sonuç olarak çalışma grubundaki hastalarda yeterli doku bütünlüğü sağlanmakta, hızlı iyileşme için uygun koşul oluşmaktadır. Aynı zamanda uterus alt segment kesisinin tamirinde harcanan süre 3 kat azalmış, kesi yerinden olan kanama da 2 kat azalmıştır (p= 0.03). Komplikasyonlar da çalışma grubunda %8,5 iken kontrol grubunda %40 olarak saptanmıştır. Kullanılan cerrahi materyal miktarı da belirgin olarak azalmıştır.

Abstract

Cesarean section describes the delivery of a fetus through a surgical incision of the anterior uterine wall. We have compared a new tecnique of closuring the lower uterine segment incision with the classic technique. Cesarean section is the most common operation in obstetrics. The cesarean delivery rate is increasing since 1970'ies and it is about 30% in recent years. Since January 1998, we repaired the lower uterine segment incisions with a new technique in our clinic. The study group has contained 82 pregnant women and the control group has contained 40 pregnant women where we used a two-layer closure of the myometrium Both groups were evaluated for operation time, complications, ultrasonography of the incision line and surgical materials that were used. As a result, the study group was decided for the fast healing and tissue completeness. With our technique, we found a threefold decrease for the operation time and twofold decrease for the bleeding from the incision line. The complication rate was 8.5 % at the study group and 40% at the control group. Surgical materials used at the operation were also significantly decreased.
