Özet

Gliomalar arasında astrositlerden köken alan ve malignite derecelendirmesinin en yüksek olduğu tümör grubu glioblastoma multiformelerdir (GBM). GBM lerin radyolojik görünümü, cerrahisi ve diğer tedavilerin prognoz üzerindeki rollerinin belirlenmesi önemlidir. Açık cerrahi ile tedavi edilen, histopatolojik tanısı GBM olan 11 olgu retrospektif olarak incelendi. Olguların bilgisayarlı tomografi (BT), manyetik rezonans (MR) görüntüleme bulguları, cerrahisi ve diğer tedavileri, takipleri birlikte değerlendirildi. Olguların 4'ü bayan, 7'si erkek, ve yaşları 15 ile 70 arasında (Ortalama 52) değişmekte idi. Lezyonların lokalizasyonları; frontal (n:1,%9), frontotemporal (n:2,%18), frontoparietal (n:2,%18), temporal (n:2,%18), temporoparietooksipital (n:1,%9), parietal (n:1,%9), ve oksipital (n:2,%18) idi. Olguların ikisi erken postoperatif dönemde kaybedildi (0-30 gün) (% 18). Diğer dokuz olguya operasyon sonrasında radyoterapi uygulandı, iki olgu da radyoterapi sonrasında kaybedildi (% 18). Kalan 7 olgu ise ortalama 10 aydır takip edilmektedir (% 64). Genel olarak GBM li olguların prognozlarının kötü olmasına rağmen; bulgu ve semptomların ortaya çıkışı ile radyolojik tanı konulması arasındaki sürenin nispeten uzun olması, lezyon içerisinde kistik komponentin bulunması, etrafındaki ödem alanının az olması, vital nöral ve vasküler yapılara uzak olmasıyla beraber operasyon öncesinde genel ve nörolojik durumunun iyi olması daha iyi prognoza sebep olabilecek özellikler olarak düşünüldü. GBM nin mümkün olduğunca çok çıkarılması ve rezidüel tümör miktarı, tümör progresyonunu ve yaşam süresini etkilemektedir.

Abstract

Among gliomas, the most malignant tumor subgroup is glioblastoma multiforme that originated from astrocytes. The role of radiological features, surgery and other treatment modalities on the prognosis of GBM patients is important. 11 patients in whom the open surgery revealed histopathological diagnosis of GBM were evaluated retrospectively. The findings of Computed tomography (CT) and magnetic resonance (MR) imaging, the surgical features, and follow-ups of the patients were assessed together. There were 4 female and 7 male patients whose ages ranged between 15 and 70 (average 52). Locations of the lesions were as follows: frontal (n:1,%9), frontotemporal (n:2,%18), frontoparietal (n:2,%18), temporal (n:2,%1 8), temporoparietooccipital (n:1,%9), parietal (n:1,%9), and occipital (n:2,%1 8). Two of the patients were dead at the early postoperative period (0-30 days) (18 %). Other 9 patients were irradiated. Other two patients were also dead after the irradiation. Remaining 7 patients have been followed for average 10 months. Although prognosis of the patients with glioblastoma multiforme is usually poor, longer period of time between the presentation and diagnosis, presence of a cystic component in the lesion, less perifocal edema, situation far from the vital neural and vascular structures, and good preoperative status were assessed as factors that may cause a relatively better prognosis. Excision of glioblastoma multiforme as much as possible and residual tumor volume effect the tumor progression and prognosis.
