Musküler distrofi tanısı almış olgularda distrofin gen mutasyon analiz sonuçları ve klinik bulgular ile korelasyonu

Özet
"X linked" kalıtım kalıbı gösteren Duchenne Musküler Distrofi ve ondan daha hafif bir klinik tablo oluşturan Becker Tip Musküler Distrofi tüm kas hastalıkları içinde önemli bir grubu oluşturmaktadır. Her ikisi de distrofin genine ait mutasyonlar ile karşımıza çıkar. Bulunan mutasyonların %65' i farklı eksonları tutan delesyonlardır. Burada anabilim dalımıza musküler distrofi ön tanısı ile gönderilen 16 olgu ve ailesi distrofin genine ait delesyonlar yönünden taranmış (pb, pm, 3, 4, 8, 12, 17, 19, 32, 34, 42, 43, 44, 45, 46, 47, 48, 49, 50, 51 ve 52. eksonlar) ve elde edilen sonuçlar klinik bulgular ile karşılaştırılarak değerlendirilmiştir. Değerlendirmeye alınan 16 olgudan birinde distrofin gen mutasyonu tanımlanmamış olup olgu halen myotonik distrofi olarak takip edilmektedir. Duchenne Musculer Distrofi ve Becker Musküler Distrofi tanısı alan olguların %55' inde distrofin genine ait mutasyon tanımlanmıştır. Mutasyon saptanan 7 olgu Duchenne Musculer Distrofi ve Becker Musküler Distrofi tanısı almış, 3 kadın olguda ise DMD taşıyıcılığı saptanmıştır. Hastalık tanısı alan tüm olgularda tanı klinik bulgular, kas biyopsisi, EMG bulguları ve biyokimya sonuçları (artmış CPK düzeyleri) ile desteklenmiştir. Mutasyon tanımlanmayan 7 olgudan biri Becker Musküler Distrofi, 6' sı ise Duchenne Musküler Distrofi tanısı almıştır. Elde edilen sonuçlar distrofin genine ait mutasyon analizinin tanı ve prenatal tanıdaki önemini göstermektedir.
Abstract
Duchenne Muscular Dystrophy and Becker Musculer Dystrophy (milder form) inherited as X linked are the important groups of muscular dystrophies. Both of these types are caused by mutations in dystrophin gene. Partial deletions of variable size are common (%65). Here we present the dystrophin gene mutation results of 16 cases and their families with the diagnosis of muscular dystrophy referred to our department (exons; pb, pm, 3, 4, 8, 12, 17, 19, 32, 34, 42, 43, 44, 45, 46, 47, 48, 49, 50, 51 and 52) and the results were correlated with the clinical findings. In one out of 16 cases who has no dystrophin gene mutation diagnosed as myotonic dystrophy. In 55% cases, diagnosed as DMD and BMD, dystrophin gene mutation had been observed. In all cases, the diagnosis were supported with clinical findings, muscle biopsy results, EMG results, biochemistry results (high CPK level). In non-mutated cases, 7 patient had been diagnosed as DMD whereas 3 women had been diagnosed as DMD carrier. The results may represent us the importance of the dystrophin gene mutations in the diagnosis and prenatal diagnosis of muscular dystropy.
