Özet
İnterlökin-10 (IL-10) antiinflamatuvar özellikleri olan bir sitokindir. Bu çalışmada bakteriyel menenjitli(n= 18), kabakulak menenjitli (n= 19) çocuklar ve kontrollerden (n= 10) elde edilen beyin-omurilik sıvısı(BOS) ve kan örneklerinde IL-10 konsantrasyonları ELISA yöntemiyle ölçülmüştür. BOS IL-10 değerlerinin,BOS protein düzeyi ve lökosit sayısıyla ilişkisi araştırılmıştır. BOS IL-10 konsantrasyonları hem bakteriyel,hem viral menenjitte anlamlı olarak yüksek bulunmuştur (sırasıyla p= 0.001 ve p= 0.02). Ayrıca bakteriyel menenjitte IL-10'un serum ortalama değerleri (48.8 ±117 pg/mL), viral menenjit (22.7 ± 46 pg/mL) ve kontrollere göre (11.7 ± 10.4 pg/mL) hafifçe yüksek bulunmuştur. Menenjitli hastalarda BOS IL-10 değerleri serum değerlerinden daha yüksek olup, aralarında korelasyon saptanmamıştır. BOS protein düzeyleri ile IL-10 düzeyleri arasında belirgin bir korelasyon saptanması (r= 0.45, p= 0.001), kan-beyin bariyeri hasarı nedeniyle IL 10'un BOS'ta yükseldiğini düşündürmektedir. Bununla birlikte yüksek BOS konsantrasyonları ve BOS lökosit sayısıyla ilişkisi, IL-10'un intratekal sentezlendiğinin de göstergesidir. Bu sonuçlara göre IL-10 meningeal inflamasyonda ve menenjit fizyopatolojisinde önemli rol oynayan bir sitokindir, fakat fonksiyonu tam olarak bilinmemektedir.
Abstract
Interleukin-10 (IL-10) is a cytokine with anti-inflammatory effects. In this study, IL-1O concentrations we re measured by using ELISA in 47 paired cerebrospinal fluid (CSF)/blood samples obtained from children with bacterial meningitis (n= 18), mumps meningitis (n= 19), and controls (n= 10). Its CSF concentrations were compared with the intrathechal leukocyte recruitment and protein levels. CSF IL-10 levels were markedly elevated in both bacterial and viral meningitis. In addition, in acute bacterial meningitis, mean serum IL-10 levels were slightly higher (48.8 ±117 pg/mL) than that in the serum samples of children with viral meningitis (22.7 ± 46 pg/mL) and than in that of controls (11.7 ± 10.4 pg/mL). CSF IL-10 levels were higher than the serum levels in the patients with meningitis and did not correlate with each other. There was a positive correlation between CSF protein and CSF IL-10 levels (r- 0.45, p- 0.001) suggesting that a damaged blood brain barrier contributed to the elevated IL-10 concentrations in CSF. However, increased IL-10 levels in CSF and correlation between CSF levels and CSF leukocyte counts also suggested intrathecal synthesis of IL 10. These results indicate that IL-10 may play an immunoregulatory role in the meningeal inflammatory net work and it is an important cytokine in meningeal physiopathology but its exact physiologic role is not known.
