Özet
Bruselloz birçok sistemi tutan ve ciddi komplikasyonlarla seyredebilen bir hastalıktır. Bu bildirimde; me¬nenjit, vertebral osteomiyelit ve ileum osteomiyelitine sekonder gelişen spontan kemik fraktürünün birlikte görüldüğü bir bruselloz olgusu sunulmuştur. Yirmibir yaşındaki kadın hasta; sağ kalçada ağrı, yürüme güçlüğü, bulantı ve kusma yakınmalarıyla kliniğimize kabul edilmiş ve yapılan incelemeler sonucu, nörobruselloz + brusella osteomiyeliti (L4) + ileum osteomiyelitine sekonder gelişen spontan asetabular köşe fraktürü saptanmıştır. Olgu, 3 ay süreyle rifampisin + doksisiklin + trimetoprim-sulfametoksazol ile tedavi edilmiş, klinik ve serolojik olarak iyileşme sağlanmıştır, ilgili 3 literatür gözden geçirilmiş ve brusella osteomiyelitine sekonder spontan kemik fraktürünün oldukça nadir görüldüğü tespit edilmiştir.
Abstract
Brucellosis is a multisystem disease and may cause severe complications. In this report, a case of brucel¬losis observed together with meningitis, vertebral osteomyelitis and spontaneous bone fracture secondary to ileum osteomyelitis is presented. A 21-year-old woman with the symptoms of right hip pain, difficulty in walking, nausea and vomiting was admitted to our clinic. After the investigations, diagnosis of neurobrucellosis + brucellar osteomyelitis (L4) + spontaneous bone fracture secondary to ileum osteomyelitis was ma¬de. The case was treated with rifampin + doxycycline + trimethoprim-sulfamethoxazole for 3 months and clinical and serological cure was attained. Related literature is reviewed and it is ascertained that spontane¬ous bone fracture secondary to brucella osteomyelitis is rarely seen.
