Standart ve lipozomal amfoterisin B' nin febril nötropenik hastalarda etkinliğinin değerlendirilmesi

Özet

Amfoterisin B geniş antifungal spektrumu nedeni ile febril nötropenik hastaların tedavisinde sık olarak kullanılmaktadır. Çalışmada 01 Ocak 1996-31 Aralık 1996 tarihleri arasında tedavileri sırasında standart veya lipozomal amfoterisin B kullanılan 47 hastada gelişmiş olan toplam 53 nötropenik ateş atağı değerlendirilmiş-tir. Onaltı hastada gelişen 18 nötropenik ateş atağı konvansiyonel, 31 hastada gelişen 35 nötropenik ateş atağı ise lipozomal amfoterisin B ile tedavi edilmiştir. Hastaların 16'sı kadın, 31'i erkek ve median yaş 33 (15-60)'tür. Antifungal tedaviye standart grupta %77.7, lipozomal grupta %60, toplam olarak %66 oranında yanıt alınmıştır. Yanıt oranları ve yanıt süreleri iki grup arasında istatistiksel olarak farklı değildir. En sık rastlanan yan etki hipopotasemidir. Hipopotasemi standart grupta %33.3, lipozomal grupta ise %34.3'tür. Altı (%I2.8) hastada nefrotoksisite görülmüştür. Bu hastalardan 3/18'i (%I6.6) konvansiyonel amfoterisin B, 3/35'i (%8.6) lipozomal amfoterisin B kullanmışlardır.

Abstract

Amphotericin 6 has a wide spectrum of activity against fungi and therefore it is preferred in febrile neutropenic patients. This study included 47 patients who developed 53 febrile neutropenic attacks between January 01,1996 and December 31,1996. Sixteen patients ( 18 febrile neutropenic attacks) were treated with conventional amphotericin B, 31 patients (35 febrile neutropenic attacks) were treated with liposomal amphotericin B. Sixteen patients were female and 31 were male. The median age was 33 (15-60) years. Response to the treatment was comparable 77.7% in the conventional group and 60% in the liposomal group. Overall response rate was 66%. The most common adverse effect was hypokalemia (33.3% in the conventional, 34.3% in the liposomal group). Six patients (12.8%) experienced nephrotoxicity (16.6% and 8.6%, respectively).
