Özet
Bu çalışmada 1991-1994 yıllarında, kliniğimizde yatırılarak izlenen 15 rekürren (yineleyen) menenjit olgusu değerlendirilmiştir. Olguların I3'ü (%87) erkek, 2'si (%I3) kadındır. Yaşları 14-57 arasında, ortalama 29.93$\pm$ 12.63 olarak bulunmuştur. Bir olgu onüçüncü, bir olgu beşinci, 2 olgu dördüncü ve 11 olgu da ikinci atak ile başvurmuşlardır. Ateş, başağrısı ve meninks irritasyon bulguları olguların tümünde gözlenmiştir. Bilinç, olguların 5'inde (%33) kapalı, 7'sinde (%47) bulanık, 3'ünde (%20) ise açıktır. Hastalar, yakınmaları başladıktan itibaren ortalama 1.7 gün sonra kliniğimize ulaşabilmişlerdir. Dördüne, gelmeden önce antibiyotik tedavisi verilmiştir. Ondört olgu septik, bir olgu ise aseptik menenjit olarak değerlendirilmiştir. Septik menenjitlerin 7'sinden (%50) Streptococcus pneumonias, birinden (%7.1) Neisseria meningitidis sorumlu bulunmuştur. Yineleyen menenjite neden olabilecek predispozan faktörler araştırıldığında 3'ünde (%20) kafa travması ve dura defekti, 2'sinde (%I3) kronik otit, 2'sinde (%I3) kronik otit ve mastoidit, 3'ünde (%20) sinüzit, birinde nazal polip operasyonu tespit edilmiş; 4 olguda(%27) ise herhangi bir neden saptanamamıştır. Hastaların tümünde aynı gün tedaviye başlanmıştır. Olguların I4'ü (%93) iyileşmiş, biri (%7) ise kaybedilmiştir. Predispozan faktör tespit edilen olgular ileri tetkik ve tedavi için Nöroşirurji veya Kulak-Burun-Boğaz kliniklerine sevk edilmişlerdir.
Abstract
In this paper, 15 cases of recurrent meningitis admitted to our department between 1991 and 1994 were evaluated. Thirteen (87%) of the patients were male and 2 (13%) were female. Mean age of the patients was 29.93$\pm$12.63 with a range from 14 to 57. One patient had the 13th, one had the fifth, two had the fo-urth and the other I I had the second episodes at admittance. All of the patients had fever, headache and meningeal irritation signs. Five (33%) of the patients were unconscious, 7 (47%) were semi-conscious and 3 (20%) were conscious. Patients were admitted to the hospital on average 1.7 days after their first compla-ints had begun. Four of them had recieved antibiotic therapy before admittance. Fourteen cases were evalu-ated as septic, one as aseptic meningitis. Seven (50%) of the septic cases were caused by Streptococcus pne-umoniae and one (7.1 %) by Neisseria meningitidis. As predisposing factors for recurrent meningitis, 3 (20%) patients had head trauma and dura defect, 2 (13%) had chronic otitis, 2 (13%) had chronic otitis and masto-iditis, 3 (20%) had sinusitis, I had nasal polyp operation and no pathological background was found for 4 (27%) patients. Treatment was started on the day of admittance. Fourteen (93%) of the patients recovered and one (7%) died. The patients having predisposig factors were sent to Neurosurgical or Ear-Nose-Throat departments for further evaluation and therapy.
