Özet

Giriş: Bu çalışmada, Ocak 2007-Mayıs 2009 tarihleri arasında Başkent Üniversitesi İstanbul Uygulama ve Araştırma Merkezi Genel Yoğun Bakım Ünitesindeki alet ilişkili infeksiyon hızları, etken mikroorganizmalar ve antibiyotik duyarlılıkları incelenmiştir. Hastalar ve Metod: Hastalar, hastaya ve laboratuvara yönelik aktif prospektif sürveyans yöntemiyle izlenmiştir. Bulgular: Çalışma döneminde takip edilen 336 hastanın 44'ünde 91 alet ilişkili infeksiyon saptandı. Ventilatörle ilişkili pnömoni hızı 30.2/1000 ventilator günü, santral kateter ilişkili kan dolaşımı infeksiyonu hızı 17.7/1000 kateter günü ve üriner kateter ilişkili üriner sistem infeksiyonu hızı 2.9/1000 üriner kateter günü olarak saptandı. Alet ilişkili infeksiyon etkeni olarak toplam 89 mikroorganizma izole edildi. İzole edilen mikroorganizmaların %48.1'i gram-negatif bakteri, %37.4'ü gram-pozitif bakteri, %12.17 Candida spp. idi. En sık tespit edilen mikroorganizmalar Staphylococcus spp. (%24.2), Pseudomonas spp. (%15.4), Enterococcus spp. (%13.2) ve Candida spp. (%12.1) olarak belirlendi. İzole edilen enterik bakterilerde siprofloksasin ve seftriakson için direnç oranları sırasıyla %77.8 ve %72.3 olarak saptandı. Pseudomonas aeruginosa izolatlarında imipenem direnci %42 saptandı. Acinetobacter spp. izolatlarında en etkili antibiyotik imipenem (direnç oranı %25) iken, diğer antibiyotiklere (seftriakson, sefepim, amikasin vb.) yüksek oranda direnç (%91.7) saptandı. Stafilokoklarda metisilin direnci %86.4 olarak bulundu. Enterokoklarda vankomisin direnci disk difüzyon yöntemi ile %50 bulundu. Sonuç: Çalışmamızda ventilatörle ilişkili pnömoni ve santral kateter ilişkili kan dolaşımı infeksiyonu oranları literatürde bildirilen oranlardan daha yüksek bulunmuştur.

Abstract

Introduction: In this study, device-associated infection rates, isolated organisms and their antibiotic susceptibilities were investigated in the General Intensive Care Unit of Başkent University Istanbul Research and Education Center from January 2007 to May 2009.Patients and Methods: Patients were followed by patient- and laboratory-based active prospective surveillance method. Results: During the study period, a total of 91 device-associated infections were detected in 44 of 336 patients. The device-associated infection rates were as follows: ventilator-associated pneumonia rate was 30.2/1000 ventilator days; central catheter-associated bloodstream infection rate was 17.7/1000 catheter days; urinary catheter-associated urinary tract infection rate was 2.9/1000 urinary catheter days. Totally, 89 microorganisms were isolated; 48.1% of them were determined as gram-negative rods while 37.4% were gram-positive cocci and 12.1% were Candida spp. The most commonly isolated microorganisms were Staphylococcus spp. (24.2%), Pseudomonas spp. (15.4%), Enterococcus spp. (13.2%), and Candida spp. (12.1%). 77.8% and 72.3% of the Enterobacteriaceae isolates were resistant to ciprofloxacin and ceftriaxone, respectively. Forty-two percent of Pseudomonas aeruginosa isolates were resistant to imipenem. İmipenem, with a resistance rate of 25%, was the most effective agent against the isolates of Acinetobacter spp.; however, the isolates were highly resistant (91.7%) to other antibiotics including ceftriaxone, cefepime and amikacin. Methicillin resistance was 86.4% in the Staphylococcus spp. isolates. Fifty percent of Enterococcus isolates were resistant to vancomycin. Conclusion: In our study, ventilator-associated pneumoniae rate and central-catheter-associated blood stream infection rate were higher than the rates reported in the related literature.
