Akut Gastroenterit Nedeniyle Acil Serviste Yatırılarak İzlenen, Rotavirus Saptanan ve Saptanmayan Çocuk Olguların Klinik ve Laboratuvar Farklılıklarının Değerlendirilmesi

Özet
Amaç: Akut gastroenteritler çocuklarda gelişmiş ve gelişmekte olan ülkelerde olduğu gibi ülkemizde de önemli bir sağlık sorunudur. Bu çalışmada, rotavirüs saptanan ve etken saptanmayan akut gastroenteritli çocuk olguların klinik ve laboratuvar verileri değerlendirildi. Gereç ve Yöntem: Çocuk Acil Kliniği’nde 2017 yılında yatırılarak takip edilen 1 ay-5 yaş arası akut gastroenteritli olgular; yaş, uyruk, cinsiyet, mevsim, yatış günü, kusma varlığı, ateş varlığı, dehidratasyon derecesi, total beyaz küre, ortalama platelet volümü, serum transaminaz, ürik asit, üre, kreatinin, sodyum, potasyum, kan gazında pH ve bikarbonat değerleri, gaita incelemeleri (kültür ve hızlı viral antijen tarama testi) değerlendirildi. Bulgular: Toplam 576 hastanın yaş ortalaması 31.77 aydı (min-max: 6-59 ay). Hastaların 270’i (%46.9) kadındı. Olguların 138’inde (%24) rotavirüs, 11’inde (%1.9) adenovirüs, 22’sinde (%3.8) paraziter enfeksiyon saptandı, 405’inde (%70.3) etken saptanamadı. Rotavirüs pozitif olgularda kusmanın daha sık, dehidratasyon derecesinin daha yüksek, yatış süresinin daha uzun, görülme yaşının daha küçük olduğu, kasım ayında diğer aylara göre daha sık görüldüğü, beyaz küre sayısının daha düşük olduğu, transaminazların ve ürik asit düzeyinin daha yüksek olduğu ve metabolik asidozun daha sık eşlik ettiği saptandı. Sonuç: Akut gastroenteritler, 5 yaş altı çocukluk yaş grubunda hastaneye başvuru ve hastaneye yatış nedenleri arasında sıktır. Rotavirüs, önemli bir kısmını oluşturur ve klinik olarak daha ağır bir seyir gösterebilmektedir.
Abstract
Objective: Acute gastroenteritis in children is a major health problem in our country as well as in developed and developing countries. In this study, clinical and laboratory differences between rotavirus detected and undetected gastroenteritis in children were evaluated. Material and Method: Patients between the ages of one month-5 years who were hospitalized in Pediatric Emergency Clinic in 2017 were evaluated retrospectively by age, nationality, sex, season, day of hospitalization, presence of vomiting, presence of fever, degree of dehydration, total white blood cell, mean platelet volume, serum transaminases, uric acid, urea, creatinine, sodium, potassium, pH and bicarbonate values in blood gas, gaita tests. Results: The mean age of 576 patients were 31,77 months (min-max:6-59 months). 270 (46.9%) of the patients were female. Of the patients, 138 (24%) had rotavirus, 11 (1.9%) had adenovirus, 22 (3.8%) had parasitic infection and 405 (70.3%) could not be detected. In patients with rotavirus positive cases; vomiting was more frequent, the degree of dehydration was higher, the duration of hospitalization was longer, the age was smaller, more frequently seen in November than in other months, the white blood cell count was lower, the transaminases and uric acid levels were higher and metabolic acidosis were more frequently accompanied. Conclusion: Acute gastroenteritis is a common cause of hospital admission and hospitalization in children under 5 years of age. Rotavirus is an important part of the disease and can be clinically more severe.
