Humerus Diafiz Kırıklarında Konservatif, İntramedüller Çivileme ve Plaklı Osteosentez Tedavi Sonuçlarının Karşılaştırılması

Özet

Amaç: Humerus diafiz kırığı nedeniyle kliniğimizde tedavi edilen hastalarda inramedüller çivileme, plaklı osteosentez ve konservatif tedavi sonuçlarının retrospektif değerlendirilmesi amaçlandı. Gereç ve Yöntem: Fırat Üniversitesi Hastanesi Ortopedi ve Travmatoloji kliniğinde 2013 Ocak-2019 Şubat arasında humerus diafiz kırığı nedeniyle konservatif, intramadüller çivileme ya da plaklı osteosentez ile tedavi edilen ve yeterli verileri olan 79 hasta çalışmaya dahil edildi. Hastaların klinik, fonksiyonel ve radyolojik bulguları retrospektif olarak incelendi. Bulgular: Yetmişdokuz hastanın 59’u erkek, 20’si kadındı. Kırıklar Arbeitsgemeinschaft für Osteosynthesefragen (AO) sınıflandırılmasına göre gruplandırıldığında 50 hasta (%63) AO tip A,15 hasta (%19) AO tip B, 14 hasta da (%18) ise AO tip C kırık mevcuttu. Hastaların 23 (%29) tanesi intamedüller çivilemeyle, 16 (%20) tanesi konservatif tedaviyle, 40 (%51) tanesi ise plaklı osteosentez ile tedavi edilmişti. 79 hastanın 73’ünde (%92) kaynama görüldü. Klinik değerlendirmede Stewart-Huntley ve QuickDASH sistemi kullanıldı. Sonuç: Stewart-Huntley skorlama sistemine göre mükemmel ve iyi sonuç olan hastaların oranı intramedüller çivilemede %74, konservatif tedavide ve plaklı osteosentezde %87 olarak bulundu. Üç tedavi yöntemi ile QuickDASH skoru arasındaki ilişkinin anlamlı olduğu görüldü.

Abstract

Objective: We aimed to retrospectively compare and evaluate the results of intramedullary nailing, plaque osteosynthesis and conservative treatment in patients treated in our clinic due to humerus diaphysis fracture. Material and Method: In Fırat University Faculty of Medicine, Clinics of Orthopedics and Traumatology, 79 patients who were treated with conservative, intramedullary nailing or plaque osteosynthesis between January 2013 and February 2019 and with sufficient data were included in the study. The clinical, functional and radiological findings of the patients were examined retrospectively. Results: Seventy-nine patients were 59 males and 20 were females. When the fractures were grouped according to Arbeitsgemeinschaft für Osteosynthesefragen (AO) classification, there were 50 patients (63%) of AO type A, 15 patients (19%) of AO type B, and 14 patients (18%) of AO type C fractures. Of the patients, 23 (29%) were treated with intamedullary nailing, 16 (20%) with conservative treatment, and 40 (51%) with plate osteosynthesis. In 73 (92%) of 79 patients, union was observed. Conclusion: According to the Stewart-Huntley scoring system, the proportion of patients with excellent and good results was 74% in intramedullary nailing, 87% in conservative treatment and plate osteosynthesis. It was observed that the relationship between the three treatment methods and the QuickDASH score was significant.
