Özet
Amaç: Çalışmanın amacı 10 ve 24. gebelik haftası arası intrauterin ölü gebelikleri değerlendirerek misoprostolün etkinliğinin doğum öyküsü, doğum şekli ve gebelik haftasına göre etkinliğini araştırmaktır. Materyal ve Metod: Hastanemize başvuran 10-24 gebelik haftası arasındaki intrauterin ölü fetuslu 60 hastanın verileri retrospektif olarak değerlendirildi. Hastaların demografik, klinik, hematolojik verileri toplandı. Bulgular: 10 ile 24 gebelik haftası arası intrauterin ölü fetüs nedeniyle gebelik terminasyonu yapılan toplam 60 hasta çalışmaya dahil edildi. Gebelik terminasyonu için tüm hastalara aynı tedavi protokolü uygulandı. Hastalar ilk trimester (10-14.hafta arası) (n=22) ve ikinci trimester (14-24 hafta arası) (n=38) olmak üzere iki gruba ayrıldı. Gruplar arasında abort öncesi ve sonrasında bakılan hemogram parametreleri ve CRP düzeyleri arasında anlamlı fark bulunmamıştır. Misoprostol uygulamasından sonra aborta kadar geçen süre ikinci trimester grubunda 15.08 ± 10.68 saat iken, ilk trimester grubunda 9.23 ± 3.99 saat olarak bulunmuş olup istatistiksel olarak anlamlı derecede uzun saptanmıştır (p=0.017). İlk trimester grubunda ikinci kür ihtiyacı, tedaviye yanıtsızlık nedeniyle intrauterin balon uygulanması ve histerotomi ihtiyacı hiç gözlenmezken, ikinci trimester grubunda 4 hastaya 2. Kür uygulanmak zorunda kalınmış, 3 hastaya intrauterin balon uygulanmış ve 2 hasta histerotomiye alınmıştır. Hastalar doğum durumlarına göre gruplara ayrıldığında ise normal doğum öyküsü olan hastaların, nullipar gebeler ve sezeryan öyküsü olan gebelere kıyasla istatistiksel olarak anlamlı derecede daha kısa sürede misoprostole yanıt verdiği görülmüştür. (p<0.044). Sonuç: İlk trimester ve ikinci trimester ölü fetusların sonlandırılmasında misoprostolün vaginal ve sublingual kombine kullanımı efektif ve güvenlidir. Ancak gebelik haftası ve doğum şekli indüksiyon sonrası abort süresinde etkili olduğundan süreç konusunda hastanın öncesinden bilgilendirilip, desteklenmesi hasta uyumunu arttıracaktır.
Abstract
Aim: The purpose of this study is to evaluate pregnancy terminations between weeks of 10 and 24, and to investigate efficiency of misoprostol according to birth history, birth type and gestational week of patients. Material and Method: We retrospectively evaluated data of 60 patients admitted to our clinic between 10- and 24 weeks of pregnancy due to intrauterine ex fetus. Demographic, clinic and hematological data of patients were collected andanalysed. Results: A total of 60 patients who underwent termination of pregnancy due to intrauterine dead fetüs between 10 and 24 weeks of gestation were included in the study. The same treatment protocol was applied to all patients for pregnancy termination. The patients were divided into two groups as first trimester (10-14 weeks) (n = 22) and second trimester (14-24 weeks) (n = 38). There was no significant difference between the groups in terms of hemogram parameters and CRP levels before and after abortion. After misoprostol administration, the duration of abortion was 15.08 ± 10.68 in the second trimester group, while it was 9.23 ± 3.99 in the first trimester group and was statistically significantly longer in the second trimester group (p = 0.017). In the first trimester group, the need for second cure, intrauterine balloon application due to medical treatment failure and the need for hysterotomy were not observed, while in the second trimester group, 4 patients were applied second cure of misoprostol, 3 patients were applied intrauterine balloon and 2 patients were underwent hysterotomy. When the patients were divided into groups according to their birth status, it was observed that patients with history of normal spontaneous vaginal delivery responded to misoprostol in a statistically significantly shorter time compared to nulliparous and patients with a history of cesarean section. (p<0.044). Conclusion: Combine duse of vaginal and sublingual misoprostol is effective and safe in termination of first trimester and second trimester intrauterine ex fetuses. However, since the gestational week and the mode of delivery are effective in the abortion period after misoprostol induction, informing and supporting the patient about the process will increase patient compliance.
