Yenidoğan sepsinin tanısında C-reaktif protein, prealbumin ve hematolojik parametrelerin değerlendirilmesi

Özet
Bu çalışmada yenidoğan sepsisinde erken tanı metodlarını değerlendirmek amacıyla. 20 sep-sisli ve 20 kontrol olgusunda CRP, prealbumin ve hematolojik parametreler çalışılmıştır. Elde edilen hematolojk parametreler lökosit sayısı, total ve immatür polimorfo nükleer lökosit sayısı, immatürelltotal ve immatürelmatür polimorfo nükleer lökosit oranı, polimorfonükleer lökosit-lerde vakuolizasyon toksik granülasyon ve Döhle cisimciklerinin mevcudiyeti ve trombosit sayısını içeren hematolojik skorlama sistemine göre değerlendirilmiştir. Hematolojik skorlama siste¬minde en az 3 parametrenin anormalliği mevcut olduğunda %90'ının üzerinde sensitivite spesifite pozitif ve negatif prediktif değerler elde edilmiştir. Sepsisli hastalarda CRP değerleri anlamlı olarak yüksek (p<0.01) ve prealbumin değerler ianlamlı olarak düşük (p<0.01) bulunurken sensitivite spesifite pozitif ve negatif prediktif değerleri hematolojik skorlama siste¬minde elde edilen değerlerin altında bulunmuş¬tur. Elde edilen bulguların değerlendirilmesi yenidoğan sepsisi tanısında en güvenilir metod olarak öncelikle hematolojik skorlama sistemi, bunları izleyerek CRP ve prealbumin değerleri olduğunu göstermiştir.
Abstract
In this study, early diagnosis of neonatal sepsis was evaluated by studying serum prealbumin and C-reaktive protein levels and complete blood count with differential diagnosis in 20 healthy and 20 septic newborn babies. A hematological scoring system was used for assessment of white blood cell count, total and immature neutrophils count, the ratio of immature to total neutrophils and immature to mature neutrophils, presence of toxic granulation, vacualisation and Dohle badies in neutrophils and thrombocyte count. The sensitivity, specificity, positive and negative predictive values of hematological scoring system were found more than 90%. Newborn with sepsis had elevated CRP levels (p<0.01) and decreased serum prealbumin levels (p>0.01) but the sensitivity, specificity, positive and negative predictive values of CRP and serum prealbumin levels were lower than those of hematological scoring system.
