Özet
Sistemik inflamatuvar yanıt sendromu (SIRS) enfeksiyoz veya nonenfeksiyoz nedenlere bağlı olarak gelişebilir. Salisilat intoksikasyonu noninfeksiyoz SIRS nedenlerinden biridir. Salisilat intoksikasyonunda tipik olarak metabolik asidoz ve respiratuar alkaloz gelişk. Kan bikarbonat düzeyi düşük olan her olgunun değerlendirilmesine, primer hiperven-tila'syonu dışlamak için arteriyel kan gazları analizi ve metabolik asidozun olası tipini belir-lemek için anyon açığının hesaplaması ile başlanmalıdır. Biz burada intraabdominal sepsis ve çoklu organ yetmezliği kliniği veren 62 yaşındaki erkek bir salisilat intoksikasyonunu olgusunu sunduk. Salisilat intoksikasyonu tanısını hipoalbuminemiye göre düzeltilmiş anyon açığı hesaplaması sonucunda saptadığımız yüksek anyon açığı nedeniyle serum salisilat düzeyinin araştırılması ile koyduk. Anestezistler salisilat intoksikasyonunun akut karın ve sepsis kliniği gösterebileceği konu-sunda uyanık olmalıdırlar. Metabolik asidozun değerlendirilmesinde, anyon açığının he-saplanmasının gerektiği unutulmamalıdır.
Abstract
The causes of Systemic İnflamattory Response Syndrome (SIRS) may be infectious or noninfectious cases. Salicylate intoxication is a noninfectious cause of SIRS. Acute intoxication typically results from intentional overdose by a young adult in a suicide attempt or through accidental poisoning by a child. Chronic salicylate intoxication typically occurs in a older adults who a taken salicylate for chronic İnflamattory condition. Salicylate intoxication is associated with metabolic acidosis and respiratory alkolosis. Evaluation of a patient with low [HCOs] should begin with arterial blood gases to exclude primary hyperventilation and with calculation of serum unmeasured anions, the "anion gap". In this case we reported the salicylate intoxication presenting as intraabdominal sepsis. An 62 years old man with salicylate intoxication presented with features of sepsis and multipl organ failure. We diagnosed this intoxication by calculation the anion gap corrected for hypoalbuminemia. Because of high anion gap metabolic acidosis we suspected from salicylate poisoning. We conclude that anesthesiologists should be aware of salicylate poisoning and consider it as a cause of medical abdomen or sepsis.
