Poland Sendromlu Hastada Ameliyathane Dışı Anestezi Uygulaması: Olgu Sunumu

Özet
Poland sendromu, pektoralis majör ve minör kaslarının doğumsal yokluğuna eşlik eden aynı taraflı göğüs ve üst ekstremite deformiteleri bütününü tanımlar. Bununla birlikte sendromun tek sabit bulgusu pektoralis majör kasının sternokostal parçasının yokluğudur. Bu sabit bulguya aynı tarafta pektoralis minör kasının yokluğu, meme dokusunun gelişmemesi, 2-5 arası kaburgaların olmaması, kardiyovasküler sistem bozuklukları ve multi sistem defektler eşlik edebilir. Bu hastalarda mevcut olan göğüs deformitesi ve ek hastalıklar sebebiyle anestezi uygulamaları esnasında paradoksal solunum, malign hipertermi ve akciğer herniasyonu riski mevcuttur. Bu olgu sunumunda 5 yaşında kız çocuğunun, tanısal amaçlı manyetik rezonans görüntüleme yapılması için uygulanan sedasyon anestezisi, güncel literatür taraması ile birlikte sunulmuştur.
Abstract
Poland syndrome is a congenital anomaly characterized by unilateral absence of the chest wall and ipsilateral upper extremity deformities with deficiency of the pectoralis major and minor muscles. Despite the large variance, the sine qua non is absence of a sternocostal portion of the pectoralis major muscle. Additionally, it can be combined with other anomalies, such as absence of pectoralis minor muscle as well as the 2nd, 3rd, 4th and 5th ribs, breast hypoplasia, abnormalities of the cardiovascular system and multiple organ system defects. In these patients, there is the risk of malignant hyperthermia, lung herniation and paradoxical respiration during anaesthesia due to thorax deformity and comorbidities. In this paper, we report a 5-year-old girl, who was administered sedation anesthesia for pediatric diagnostic magnetic resonance imaging, with the review of the literature.
