Özet
Amaç; Mastoidektomi sırasında karşılaşılan anatomik varyasyonları değerlendirerek daha etkin bir mastoidektomi ile hastaya daha faydalı olabilmek. Metod; Ocak 2008 ve şubat 2010 tarihleri arasında mastoidektomi yapılan 297 hastanın ameliyat bilgileri retrospektif olarak incelendi. Bulgular; Hastalardan 8(%2.6) tanesinde korner septumu nedeniyle antruma ulaşılmakta zorluk çekilmişti, 3(%1) tanesinde fasial sinir timpanik segmentte açıklık vardı ve fasiyal sinir daha fazla risk altındaydı,4(%1.3) hastada tegmen timpanideki dehissans nedeniyle dura açıktaydı ve 3(%1) hastada da dura aşağı yerleşimliydi mastedektomide üst sınırda daha fazla dikkat gerektiriyordu, 3(%1) hastada da sigmoid ven anteriosüperiorda yerleşimli olduğu için sigmoid ven mastoidektomiyi zorlaştırıyordu. Sonuç; Mastoiektomi kemikteki anotomik anomaliler ortalama %1-2.6 arasındadır. Operasyon sırasında anatomik anomaliler açısından da dikkatli olmak ameliyat sırasındaki başarımızı artıracaktır.
Abstract
OBJECTIVE: To evaluate the anatomical variations encountered during mastoidectomy in order to perform a more effective surgical intervention. METHODS: The operation details of the 297 patients who undergone mastoidectomy between January 2008 and February 2010 were investigated retrospectively. RESULTS: In 8 (2.6%) patients, the entrance to mastoid antrum was troublesome due to Korner septum. In three patients a dehiscence in the bony canal were noted which proned these patients to possible facial nerve injury. In 4 (1.3%) patients, dura was open to surgical cavity due to dehiscence of the tegmen timpany and in 3 (1%) patients, dura was as below location, mastoidectomy needed more attention at the upper border. In 3 patients the dura was inferiorly localized which necessitates much more attention at the upper border of surgical area. In 3 (1%) patients, sigmoid sinus made difficult the mastoidectomy due to anterosuperior localization of the sigmoid. CONCLUSION: The prevalence of anatomical anomalies of the mastoid bone was reported between 1–2.6%. As the awareness of the surgeon increases about the possible anatomical anomalies of mastoid bone, the increase in surgical success will follow.
