Doyle Tip 4b mallet kırıklarının pull-out sütur ve transartiküler kirschner teli fiksasyon yöntemiyle tedavisi

Özet
Amaç: Bu çalışmada Doyle sınıfamasına göre tip 4b mallet kırığı tedavisinde pull-out sütur ve transartiküler Kirschner teli ile fksasyon yönteminin sonuçları değerlendirildi. Yöntemler: Ocak 2005-Ağustos 2013 tarihleri arasında mallet kırığı nedeniyle opere edilen 15 hasta çalışmaya dahil edildi. Hastaların onu erkek, beşi kadın ve ortalama yaş 36,3 (dağılım, 15-55) idi. Cerrahide açık redüksiyonun ardından pull-out sütur ve transartiküler Kirschner teli ile fksasyon yapıldı. Hastalar Crawford değerlendirme kriterleri doğrultusunda fonksiyonel ve klinik açıdan değerlendirildi. Ortalama takip süresi 39,3 ay (dağılım, 6-99) olarak tespit edildi. Bulgular: Radyografk olarak tüm hastalarda kaynama sağlandı. Klinik olarak hiçbir hastada teldibi enfeksiyonu, cilt nekrozu, avasküler nekroz ve tırnak deformitesi gözlenmemekle birlikte distal interfalangeal eklemde daralma, dejenerasyon veya subluksasyon tespit edilmedi. Crawford değerlendirme kriterlerine göre olguların 12sinde (%80) mükemmel, iki sinde (%13) iyi ve birinde (%7) orta sonuç elde edildi. Herhangi bir açıda feksiyon kaybı izlenmezken ortalama 1,5 °C (dağılım, 0-15) ekstansiyon kaybı gözlendi. Sonuç: Hem cerrah hem de hasta memnuniyeti açısından değerlendirildiğinde mallet kırığı tedavisinde pull-out sütur ve transartiküler Kirschner teli uygulamasının, tatminkar ve önerilebilinir bir cerrahi teknik olduğu görüşündeyiz. Abstract
Aim: In this study, we evaluated the results of the pull-out suture and transarticular Kirschner wire fxation technique in the treatment of Doyle type 4b mallet fractures. Methods: The study involved 15 patients (10 males, 5 females) with a mean age of 36.3 years (range: 15-55 years) who underwent surgery for mallet fracture between January 2005 and August 2013. Following the open reduction, pull-out suture and fxation with transarticular Kirschner wire were performed as the surgical method. The patients were assessed functionally and clinically according to the Crawford criteria. The mean follow-up period was 39.3 months (range: 6-99). Results: Radiographic union was achieved in all patients. None of the patients developed infection, skin necrosis, avascular necrosis, nail deformity, and degeneration or subluxation of the distal interphalangeal joint. According to the Crawford criteria, the results were excellent in 12 (80%) patients, good in two (13%) patients and moderate in one (7%) patient. None of the patients developed fexion loss and the mean extension loss was 1.5 °C (range: 0-15). Conclusion: Considering the satisfaction of both the patient and the surgeon, we suggest pull-out suturing and transarticular Kirschner wire application which is a satisfactory surgical method in the treatment of Doyle type 4b mallet fractures.
