Osteoid osteomada cerrahi tedavi sonuçlarımız

Özet
Amaç: Osteoid osteoma (OO) kemiğin sık görülen benign kemik tümörlerinden biridir. Tedavide amaç nidusun çıkarılması ve ağrının giderilmesidir. Yazımızda tolere edilemeyen ağrılı osteoid osteomalı hastalara uyguladığımız cerrahi tedavi sonuçlarının incelenmesi amaçlanmıştır. Yöntemler: Kliniğimizde OO tanısıyla cerrahi tedavi uyguladığımız ve son kontrolleri yapılan 39 hasta çalışmaya dahil edildi. Hastaların klinik özellikleri, radyolojik bulguları, cerrahi tedavi yöntemi ve başarısı incelendi. Bulgular: Otuz dokuz hastanın 24ü erkek, 15i kadın ve yaş ortalaması 19,6 yıl olarak bulundu. En sık femur (17) ve tibiada (11) tutulum tesbit edildi. Direkt grafde 39 hastadan 18inde, bilgisayarlı tomograf (BT) çekilen 32 hastadan 28inde, manyetik rezonans görüntüleme (MRG) ile incelenen 24 hastadan 11inde nidus gösterildi. Kemik sintigrafsi çekilen 18 hastadan 10unda nidus tesbit edildi. Otuz dokuz hastadan dördünde nüks görüldü, üçüne tekrar cerrahi tedavi uygulandı. Sonuç: Tanı için anamnezde ağrının özelliklerinden ve radyolojik olarak özellikle ince kesit BTde nidusun görüntülenmesinden faydalanılmalıdır. Osteoid osteoma tedavisinde nidusun tamamen çıkarılması gereklidir. Tedavinin başarılı olduğu ağrının geçmesi ve radyolojik olarak nidusun çıkarıldığının gösterilmesi ile mümkündür. Abstract
Aim: Osteoid osteoma (OO) is one of the most common benign tumors of the bone. Treatment of choice is usually the resection of the nidus. In this study, we assessed the surgical treatment results in patients with OO who were experiencing severe pain. Methods: We included 39 patients with OO who were treated by surgical method in our clinic. Clinical features, radiological fndings, surgical method and the success of the treatment were assessed. Results: Twenty-four patients were male and 15 patients were female. The mean age of the subjects was 19.6 years. The most common sites were the femur (17) and tibia (11). Nidus was detected by plain radiography in 18 of 39 patients, by computed tomography in 28 of 32 patients, by magnetic resonance imaging in 11 of 24 patients, by bone scintigraphy in 10 of 18 patients. Relapse was seen in four of 39 patients and for three of them, surgical curettage was performed. Conclusion: The character of the pain and thin-section computed tomography must be evaluated together the make the diagnosis. The nidus must be removed completely in the treatment of osteoid osteoma. The success of the treatment is defned both by the relief of the pain and no nidus seen on radiological examinations.
