Non Hodgkin Lenfoma Nedeni İle Splenektomi Uygulanan Hastaların Retrospektif Analizi

Özet
Amaç: Non Hodgkin Lenfoma (NHL) lenfoid sistemin proliferatif bir hastalığıdır. NHL cerrahiyi, subtip analizini,postoperatif komplikasyon ve sağ kalımı literatür eşliğinde tartışmayı amaçladık. Gereç ve Yöntem: Splenektomi operasyonu sonrası histopatolojik inceleme ile NHL tanısı alan hastalar çalışmaya dahil edildi. 490 splenektomi hastası arasından 26 NHL tanısı alan hasta bulundu. Bulgular: Hastaların ortalama yaşı 62,03 (32-83) olarak tespit edildi. 26 hastanın 10’u (%38,4) erkek,16’sı (%61,6) kadın cinsiyette idi. Sekiz hasta hipersplenizme bağlı sitopeni, 3 hasta kemik iliği biyopsisi ve lenf bezi biyopsisi ile tanı konulamadığı için, 15 hasta masif splenomegaliye bağlı semptomlar nedeni ile opere edildi. Postoperatif dönemde 8 hastada komplikasyon görüldü. Bu komplikasyonlar diyafragma laserasyonu (n=1), pankreatik fistül(n=1), batın içi seroma(n=2), atelektazi(n=3) ve yara yeri enfeksiyonu(n=3) gözlendi. Patolojik incelemede 18 hasta Diffüz B hücreli lenfoma, 1 hasta Foliküler lenfoma, 1 hasta Hairy cell lösemi ve 6 hasta splenik marjinal zonelenfoma tanısı aldı. Uzun dönem takiplerinde 14 hastanın Hematoloji Onkoloji poliklinik takibinin tam iyileşme nedeni ile sonlandırıldığı, 2 hastanın lenfoproliferatif hastalık nedeni ile exitus olduğu 2 hastanın NHL dışı nedenle exitus olduğu ve 8 hastanın Hematoloji Onkoloji poliklinik takibinin devam ettiği belirlendi. Ortalama dalak ağırlığı 680 gram (340-1850) olarak hesaplandı. Sonuç: NHL tanılı hastalarda küratif veya semptomatik amaçlı, tanı konulamayan ve dalak tutulumu mevcut olan şüpheli hastalarda splenektomi uygulanması güncel tanı/tedavi modaliteleri arasında yerini korumaktadır.
Abstract
Objective: Non Hodgkin Lymphoma (NHL) is a proliferative disease of the lymphoid system. We aimed to discuss NHL surgery, subtype analysis, postoperative complications and survival in the light of the literature. Material and Method: Patients diagnosed with NHL after histopathological examination after splenectomy were included in the study. Among the 490 splenectomy patients, 26 patients diagnosed with NHL were found. Results: The meanage of the patients was 62.03 (range, 32-83). 10 (38.4%) of the 26 patients were male and 16 (61.6%) were female. Since 8 patients could not be diagnosed with cytopenia due to hypersplenism, 3 patients could not be diagnosed by bone marrow biopsy and lymph node biopsy, 15 patients were operated for symptoms due to massive splenomegaly. Complications were observed in 8 patients in the postoperative period. The secomplications were diaphragmatic laceration (n = 1), pancreatic fistula (n = 1), intra abdominal seroma (n = 2), atelectasis (n = 3), and wound infection (n = 3). In the pathological examination, 18 patients were diagnosed with Diffuse B-celllymphoma, 1 patient with Follicular lymphoma, 1 patient with Hairycellleukemia, and 6 patients with splenic marginal zone lymphoma. In the long-term follow-up, it was determined that the Hematology Oncology Clinic follow-up of 14 patients was terminated due to complete recovery, 2 patients died due to lymphoproliferative disease, 2 patients died due to non-NHL, and 8 patients were undergoing Hematology Oncology clinical follow-up. The mean spleen weight was calculated to be 680 grams (range, 340-1850). Conclusion: Splenectomy in patients diagnosed with NHL for curative or symptomatic purposes, and in suspected patients with splenic in volvement, remains among the current diagnosis / treatment modalities.
