İntradermal uygulamada hepatit B aşısının etkinliği

Özet

Bu çalışmada, hepatit B (HB) aşısının intradermal (İD) uygulanımının etkinliği, intramusküler (İM) uygulama ile karşılaştırmalı olarak araştırıldı. Çalışma grubu tıp fakültesi öğrencilerinden oluşturuldu. Uygulama iki şekilde yapıldı; (I) Klasik şema; 0., I., 6. aylarda 20 ug intramusküler (İM) aşı uygulanımı ve (2) Düşük dozlu şema; 0., I., 2., 6. aylarda 2 ug intradermal (İD) aşı uygulanımı. ikiyüzkırkaltı öğrenciye I. şema uygulanırken, gönüllü 65 öğrenciye 2. şema uygulandı. Aşılama sonrasında antiHBs antikor yanıtı "enzyme immuno assay (EIA)" ile kantitatif olarak ölçüldü. Klasik şemada 2. aşıdan en az 20 gün sonra, 2. şemada ise 3. ve 4. aşıdan en az 20 gün sonra antiHBs antikoru araştırıldı. Klasik şema uygulanan grupta, İD aşı uygulanan gruba göre daha yüksek antikor saptanmakla birlikte aşı programı tamamlandığında antikor düzeyleri aynı bulundu. Sigara içme hepatit B aşılamasında kötü yanıt nedeni olarak belirlendi. En sık saptanan yan etki enjeksiyon bölgesinde ağrı idi. Sonuç olarak, İD düşük dozda aşılama şemasının ekonomik yararları nedeniyle kullanılabilir olduğu, ancak rutine geçebilmesi için destekleyici ilave araştırmaların yapılması gerektiği düşünüldü.

Abstract

In this study, the efficacy of intradermal Hepatitis B vaccination was compared with intramuscular route. Students of medical faculty were included to the study. Vaccine was administered in two different forms. In the classical schedule 20 ug hepatitis B vaccine was administered on 0, I and 6th months by intramuscular (1M) route. In the comparative low dose schedule 2 ug hepatitis B vaccine was administered on 0, 1,2 and 12th months by intradermal (ID) route. Twohundred and fortysix students received the first schedule and sixtyfive students received the second schedule. The immune response to vaccination was measured quantitatively by enzyme immune assay (EIA). An tiHBs antibody levels was measured after 20th day of second vaccination in the first schedule. AntiHBs anti body levels were measured after.20* day of third and fourth vaccination in the second schedule. AntiHBs antibody levels were found higher in the first group than the second group who received low dose intrader mal vaccination schedule. However ID vaccination was found as effective as classical 1M schedule at the end ofvaccination.Smoking was found as a negative factor to the response to Hepatitis B vaccination. The most frequent adverse effect was pain on the injection site. In conclusion, low dose ID vaccination schedule seems to be useful for vaccination of large groups because of it's economical benefits. But it requires further investigations.
