Özet
1998-1999 yılları arasında Ağrı Asker Hastanesi, İnfeksiyon Hastalıkları Polikliniği ile Ağrı Devlet Hasta¬nesi, Dahiliye Poliklinikleri'ne başvuran 27 şarbonlu hasta prospektif olarak incelendi. Olguların I2'si erkek, 15'i bayan ve yaş ortalaması 31.5 (I I-59 yaş) idi. Bir olgu dışında olgularımızın tamamı hayvancılık ve çiftçilik¬le uğraşan ve infekte hayvanlarla teması bulunan bireylerdi. Beş olguda (% 18.5) lezyonlar malign ödem şeklin¬de olup, bir tanesi suborbital bölgede, bir tanesi alt ekstremitede, üç tanesi ise üst ekstremitedeydi. Olgula¬rın 22'sinde (%8I.5) lezyonlar malign püstül şeklinde ve bir tanesi alt ekstremitede diğerleri ise üst ekstremi¬tedeydi. Tanı 13 (%48.2) olguda bakteri izolasyonu, 8 (%29.6) olguda lezyon materyalinden direkt inceleme, 6 (%22.2) olguda ise lezyon görünümü ile konuldu, izole edilen Bacillus anthracis suşları penisilin, eritromisin, sefoksitin, sefotaksim, sefadroksil ve imipeneme duyarlı olarak bulundu. Üç suşda kloramfenikol, 2 suşda tet-rasiklin direnci saptandı. Tedavide 25 (%92.6) olguda penisilin, 2 (%7.4) olguda ise sulbaktam-ampisilin kulla¬nıldı. Suborbital lezyonu bulunan olgu, solunum problemlerinin başlaması üzerine daha ileri bir merkeze sevk edildi ve tedavi sonunda tüm olgularımızda tam şifa sağlandı.
Abstract
Twenty seven patients with anthrax applying to Infectious Diseases Department of Ağrı Military Hospi¬tal and Internal Medicine Department of State Hospital between 1998-1999 were evaluated prospectively. Twelve of them were males while females made up 15 cases. The mean age was 31.5(11 -59). Excluding one case occupied in the army, all patients were farmers having close relationship with the infected animals. In 5 (18.5%) cases, the lesions were presented as malignant edema distributed to face in one patient, observed at the upper extremity on the other one, and the remaining three cases presented lower extremity findings. In 22 (81.5%) patients, the lesions were in malign pustular form and all were on upper extremities except one lower extremity presentation. The diagnosis was confirmed by bacterial isolation in 13 (48.2%) cases, by di¬rect examination from the lesion material in 8 (29.6%) patients and in remaining 6 (22.2%) cases the diagno¬sis was established by the lesion presentation. The isolated Bacillus anthracis strains were susceptible to pe¬nicillin, eritromycin, cephoxitin, cephotaxim, cephadroxil and imipenem. Three strains were chlorampheni¬col resistant and two isolates were tetracycline resistant. For the treatment, penicillin was given to 23 pati¬ents (92.6%) while 2 cases (7.4%) were administered sulbactam-ampicilline. With emerging respiratory prob¬lems of a face restricted lesioned case, that patient was transferred to a more sophisticated center for furt¬her investigation and treatment.
