Özet

Otitis eksterna, genelde sıcak ve nemli iklimlerde görülür. En önemli etken lokal travmadır. Kulağın tahrişi, dış kulak kanalı'nın kirli havluyla aşırı kurulanması, uygun olmayan biçimde dış kulak kanalı lavajı etken organizmanın yerleşmesine zemin hazırlar. Otitis eksternadan sorumlu bakteriler genelde Pseudomonas aeruginosa, Bacillus proteus, Staphylococcus aureus'tur. Dış kulak kanalının normal mikrobik florası enfeksiyon sırasında kaybolur. Olgumuzda, altı yaşındaki kız hasta, dış kulak yolu iltihabı nedeniyle tedavi edilmekteyken ateş ve vücutta döküntü nedeniyle hastaneye yatırıldı. Aynı akşam yüzünde ve burnunda büllöz kabarcıklar oluştu. Takip eden ikinci günde lezyonlar sefuroksim tedavisine rağmen yaygınlaştı. Nikolsky belirtisi müspet olan olgunun, cilt biyopsisi yapılarak stafilokoksik haşlanmış deri sendromu teşhisi konulmuştur. Uzun süreli ve sık su teması en önemli predispozan faktördür. Hastanın tatil amacıyla bölgemizde bulunmasından dolayı bolca havuza girmiş olmasının bu enfeksiyonda tetikleyici bir etken olduğu düşünülmüştür.

Abstract

Otitis Externa is generally seen in hot and warm climates. The most important etiology is local trauma. Mechanical irritation of the ear, excessive drying of the ear channel with a dirty towel, and inappropriate drainage of the ear channel cause a tendency for establishing the responsible microorganism. Pseudomonas aeruginosa, Bacillus proteus and, Staphylococcus aureus are the bacteria responsible for otitis externa. Normal microbic flora of the outer ear channel are lost during infection. In our case, a 6 year old girl developed fever and exanthema during treatment for otitis externa and was hospitalized. Bullous lesions were seen on the face and nose during the same evening. On the following day, the lesions spread despite cefuroxime treatment. The Nikolsy sign and skin biopsy led to the diagnosis of staphylococcus scalded skin syndrome. Long term and frequent water contact is most important causal factor. The reason for this infection is thought to be that thepatient had been to a swimming pool many times during vacation.
