Özet

Amaç: Bu çalışmada, İstanbul’da bir özel hastanede toplum kökenli üriner sistem enfeksiyonlarından izole edilen Escherichia coli suşlarında antimik- robiyal duyarlılıklarının belirlenmesi amaçlanmıştır. Yöntemler: Üroloji polikliniğine 1 Ocak 2008-31 Aralık 2008 tarihleri arasında ayaktan üriner sistem enfeksiyonu şüphesi ile başvuran ve idrar örnekle- rinde üreme saptanan hastaların kültür sonuçları retrospektif olarak değerlendirildi. Bulgular: İzole edilen 191 E. coli suşunda disk difüzyon ve/veya otomatize sistem testleri ile trimetoprim-sulfametoksazol (TMP-SMX) (%40), siprof- loksasin (%19.8), seftriakson (%19.2) ve nitrofurantoin (%12.4), karşı direnç oranları saptandı. İzole edilen 191 E. coli’den 28’inde (%14.6) GSBL pozitif bulundu. Sonuç: Ampirik tedavi seçiminde, bölgesel antimikrobiyal direnç oranlarının bilinmesi, antibiyotiklerin etkin kullanılmasına katkıda bulunacaktır.

Abstract

Objective: The aim of this study was to obtain data on susceptibility patterns of Escherichia coli strains responsible for community acquired urinary tract infections in a private hospital in Istanbul, Turkey. Methods: Between the dates of 1 January 2008 and 31 December 2008 patients admitted to the urology outpatient department with suspected uri- nary tract infection and urine samples of patients with reproductive culture results were evaluated retrospectively. Results: A total of 191 E. coli strains isolated from urine cultures which were processed between January 2008 and December 2008 in the clinical labo- ratory, were included in the study. Identification of bacteria and antibiotic susceptibility tests were performed by disk diffusion and automated system. Of E. coli strains, 14.6% (n=28) were found positive for ESBL. The resistance rates of E. coli strains were found as follows; 12.4% for nitrofurantoin, 40% for trimethoprim/sulfamethoxazole (TMP-SMX), 19.8% for ciprofloxacin and 19.2% for ceftriaxon. Conclusion: Empirical therapy should be based on local antimicrobial resistance monitoring in order to optimize the use of these drugs.
