Sepsis tablosundaki immünkompetan bir hastada gelişen Herpes simpleks özefajiti

Özet

Herpes simpleks özefajiti, genelde immünsüpresif hastalarda görülmesine karşın, nadiren sağlıklı kişilerde de bildirilmiştir. Biz, akut piyelonefrit-sepsis tanısıyla izlenen, odinofaji gelişen immünkompetan bir hastada herpes özefajiti tanısı koyduk. Parenteral asiklovir tedavisiyle, hastanın odinofajisi hızla düzeldi. Herpes özefajiti tanısını tipik endoskopik bulgular ve özofagustan alınan biyopsi örneğinde PCR ile HSV-DNA pozitifliğine dayanarak koyduk. Hastamızda özefajit gelişiminde predispozan faktörün sepsise sekonder geçici hücresel immündisfonksiyon olduğunu düşünüyoruz. İlave kolaylaştırıcı bir faktör olmasa bile, sepsisli hastalarda herpes özefajiti gelişebileceği sonucuna varabiliriz.

Abstract

Herpes simplex esophagitis is generally seen in immunosuppressive patients; however, it has rarely been encountered in healthy subjects. We diagnosed herpes esophagitis in an immunocompetent patient with acute pyelonephritis-sepsis who developed odynophagia. The patients odynophagia recovered rapidly after parenteral acyclovir therapy. We diagnosed herpes esophagitis based on the typical endoscopic findings and the detection of HSV DNA by PCR in the esophageal biopsy specimen. We think that the predisposing factor for the development of esophagitis in our patient was temporary cellular immune dysfunction secondary to sepsis. We conclude that herpes esophagitis may develop in patients with sepsis without any other predisposing factor.
