Glokomda ex-press shunt implantasyon cerrahisinin göz içi basıncına etkisi

Özet

Amaç: Çeşitli glokom tiplerinde Ex-Press shunt implantasyon cerrahisinin göz içi basıncı (GİB) üzerine etkisinin de- ğerlendirilmesi. Gereç ve Yöntem: Bu geriye dönük çalışmaya 9 neovasküler, 6 psödoeksfolyatif, 5 kronik açı kapanması, 1 keratoplasti sonrası, 1 üveitik, 2 silikona bağlı, 2 primer açık açılı, 2 hayalet hücreli, 1 afakik enflamatuvar ve 1 travmatik glokomu olan 30 göz dahil edildi. Gözlerin 10’unda diyabetik retinopati, 3’ünde ön üveit öyküsü, 1’inde geçirilmiş retina dekolmanı cer- rahisi, 1’inde travmatik retina dekomanı, 1’inde travmatik vitreus hemarojisi ve lens dislokasyonu, 3’ünde travma öyküsü, 1’inde santral retinal ven oklüzyonu, 1’inde dejeneratif miyopi mevcuttu. Yedi göze (%23) eşzamanlı fakoemulsifikasyon cerrahisi yapıldı. Glokom ilacı kullanmadan GİB≤18 mmHg olması tam başarı, glokom ilacı kullanarak GİB≤18 mmHg olması sınırlı başarı, glokom ilaçlarına rağmen GİB>18 mmHg olması veya ek glokom cerrahisi gerekmesi başarısızlık ola- rak değerlendirildi. Veriler istatistiksel olarak analiz edildi. Bulgular: Hastaların ortalama takip süresi 12.4±7.1 (1-30) aydı. Ameliyat öncesi ortalama 39.3±12.8 mmHg olan GİB 16.4±10.8 mmHg’ya geriledi (p<0.000). 17 (%57) gözde tam başarı, 7 (%23) gözde sınırlı başarı, 6 (%20) gözde başarısız- lık elde edildi. Ortalama görme keskinliği (ETDRS) ameliyat öncesi 4/20±4/40 (n=16), ameliyat sonrası 4/10±4/20 (n=13) saptandı. Ortalama kullanılan antiglokomatöz ilaç kutu sayısı 2.7±0.4 kutudan 0.7±0.9 kutuya indi (p<0.000). Ameliyat sonrası dönemde 3 (%10) gözde vitreus hemorojisi, 6 (%20) gözde hifema, 9 (%30) gözde koroid dekolmanı, 1 (%3) gözde bleb disestezisi gelişti. Sonuç: Ex-Press shunt implantasyonu çoğu komplike glokomdan oluşan gözlerde ortalama 12 aylık takip sonrasında GİB’nın kontrolü açısından %80 oranında tam veya sınırlı başarı gösterebilmektedir.

Abstract

Purpose: To test the intraocular pressure (IOP) effect of the Ex-Press shunt implantation surgery in various types of glaucoma. Materials and Methods: In this retrospective study 30 eyes of 29 patients with neovascular, pseudoexfoliative, chronic angle-closure, post-keratoplasty, üveitic, due to silicone, primary open-angle, ghost-cell, traumatic aphakic, inflammatory glaucoma were included. The eyes were complicated with diabetic retinopathy in 10 eyes, a history of anterior uveitis in 3 eyes, the previous retinal detachment surgery in 1 eye, traumatic retinal detachment in 1 eye, traumatic vitreous hemor- rhage and lens dislocation in 1 eye, trauma in 3 eyes, central retinal vein occlusion in 1 eye and degenerative myopia in 1 eye. Phacoemulsification surgery was performed to 7 eyes (23%) simultaneously. Complete success was defined as IOP&#8804;18 mmHg without glaucoma medications, limited success was defined as IOP&#8804;18 mmHg using glaucoma medications, failure was defined as despite the glaucoma medications IOP>18 mmHg or additional glaucoma surgery is required to be assessed. Results: The average follow-up was 12.4±7.1 (range 1-30) months. The mean preoperative IOP was 39.3±12.8 mmHg and significantly decreased to 16.4±10.8 mmHg in postoperative period. (p<0.000). At 12 months folllow up complete success rates were 57%, limited success were 23%, failure were 20%. Conclusion: Ex-Press shunt implantation surgery had 80% full or limited IOP control rate success in most complicated glaucoma eyes with an average of 12 months follow-up.
