Özet

Amaç: Oluşturulan deney grupları ile sıçan femurunda siyatik sinir defektinin kırık iyileşmesine olan etkisi ile birlikte hiperbarik oksijen (HBO) tedavisinin sinir defekti varlığında kırık iyileşmesi üzerine olan etkisi araştırıldı. Gereç ve Yöntem: Yirmi bir adet Wistar albino cinsi sıçan üç gruba randomize edildi. Kontrol grubundaki sıçanlara Grup 1 (n=7) femur kırığı oluşturulup intramedüller tespit uygulanırken, Grup 2 (n=7) sıçanlarda femur kırığı ve intramedüller tespit sonrası siyatik sinir defekti oluşturuldu, Grup 3 (n=7) sıçanlarda ise femur kırığı, intramedüller tespit ve siyatik sinir defekti sonrası HBO tedavisi uygulandı. Dördüncü hafta sonunda sıçanların kırık femurlarının kallus alanları ölçülüp biyomekanik test yöntemi ile incelendi. Bulgular: Üçüncü grubun kallus dayanımı ortalaması, 2. grup kallus dayanımı ortalamasından yüksek olup istatistiksel olarak anlamlı fark bulunmuştur (p<0.05). 2. grup kallus dayanımı ortalamasının, 1. grup kallus dayanımı ortalamasından düşük olduğu ve bu farklılığın istatistiksel açıdan anlamlı olduğu saptanmıştır (p<0.05). 1.grup kırık kemiklerin sertlik, elastisite modülü, enerji depolama kabiliyeti, burulma momenti ve eğilme momenti ortalama değerleri, 2.grup kırık kemiklerinkinden yüksek olmakla birlikte istatistiksel olarak anlamlı farklılık; sertlik, burulma momenti ve eğilme momenti ortalama değerleri arasında bulunmuştur (p<0.05). Sonuç: Sinir yaralanması kırıkta mekanik olarak dayanıksız ve güçsüz kallus oluşumuna sebep olmaktadır. HBO tedavisi ile sinir yaralanmasının, mekanik özellikleri de dahil olmak üzere kırık iyileşmesi üzerine olumsuz etkisi sınırlanabilmektedir.

Abstract

Objective: In this study, the effect of the sciatic nerve defect in rat femurs on the improvement of the fracture, as well as the effect of hyperbaric oxygen therapy (HBO) on the healing of fracture in the presence of a nervous defect was investigated in pooled experimental groups. Materials and Methods: Twenty-one Wistar albino rats were randomized into three groups. In a control group of rats (Group 1) (n=7), a femur fracture was formed and intramedullary fixation was applied. In Group 2, (n=7) the sciatic nerve defect in rats was formed after a femur fracture and intramedullary fixation was performed In Group 3 (n=7) HBO therapy was administered in rats after a femur fracture, intramedullary fixation and sciatic nerve defect were formed. At the end of four weeks the callus areas of fractured femurs were measured and examined through biomechanical test method. Results: The mean of the callus strength in group 3 was higher than in group 2 and a statistically significant difference was found between them (p>0.05). The mean of the callus area in group 2 was lower than in group 1, therefore this difference was statistically significant (p<0.05). Although the mean values of stiffness, elasticity module, energy storing capacity, twisting moment and bending moment in the broken bones of group 1 were higher than in the broken bones of group 2, a statistically significant difference was found among them in terms of mean values of stiffness, twisting moment and bending moment (p<0.05). Conclusion: Nerve injury causes formation of mechanically nondurable and weak callus in a fracture. Nerve injury with HBO therapy, including its mechanical features, has a negative effect on improvement of the fracture.
