Anterior servikal füzyonu takiben komşu segment hastalığı gelişiminde risk faktörleri

Özet

Amaç: Bu çalışmanın amacı anterior servikal diskektomi ve füzyonun (ASDF) komşu seviye dejenerasyonuna olan etkisini klinik ve radyolojik olarak değerlendirmektir. Yöntemler: ASDF yaptığımız 89 hasta (55 kadın, 34 erkek) geriye dönük olarak değerlendirildi. Yeni osteoft oluşumu, mevcut osteoftin büyümesi, anterior longitudinal ligament ossifkasyonu varlığı, intervertebral disk mesafesinde daralma, sagital dizilim ve ROM açıları seri radyografler ve MRI ile değerlendirilerek komşu segment dejenerasyon bulguları bakıldı. Bulgular: İncelediğimiz 89 hastanın yaş ortalaması 41,3 (24-76) ay, ortalama takip süresi 34,3 (12-64) ay idi. Hastaların 12’sinde (%13,4) radyolojik komşu segment dejenerasyon bulguları görüldü. Dokuz (%75) hastada yeni gelişen şikayetler mevcuttu. Dejeneratif değişiklikler olan hastaların 7’si (%58) erkek, 5’i (%42) kadın, yaş ortalaması ise 46 (30-62) idi. Füzyon yapılan seviyenin ve kaç mesafe yapıldığının komşu segment dejenerasyonunu arttırmadığı görüldü. On iki hastanın tamamında servikal omurganın non-lordotik yapıda olduğu ve ROM’un arttığı izlendi. Sonuç: Anterior servikal diskektomi ve füzyon yapılan mesafeye komşu seviyede dejenerasyon gelişimi komşu olmayan seviyelere göre yüksektir. Füzyon yapılan mesafenin ve kaç seviye füzyon yapıldığının dejenerasyon gelişimine etkisi yoktur. (Ha se ki TIp Bül te ni 2015; 53: 120-3)

Abstract

Aim: The aim of this study was to clinically and radiologically evaluate the effcacy of anterior cervical discectomy and fusion (ACDF) in the treatment of adjacent level degeneration. Methods: We retrospectively evaluated 89 patients (55 females, 34 males) who underwent ACDF. Adjacent segment degeneration fndings were evaluated by investigating new osteophyte formation, growth of existing osteophytes, ossifcation of the anterior longitudinal ligament, presence of intervertebral disc space narrowing, sagittal alignment and range of motion (ROM) using serial radiographs and magnetic resonance imaging. Results: The mean age of the 89 patients was 41.3 (24-76) years. The mean follow-up duration was 34.3 (12-64) months. Radiographic evidence of adjacent segment degeneration was observed in 12 patients (13.4%). Nine (75%) patients had new complaints. Of the patients who had degenerative changes, 7 were (58%) were male, 5 (42%) were female; the mean age was 46 (30- 62) years. It was observed that the level of fusion and the number of fusion did not increase the adjacent segment degeneration. All of 12 patients were observed to have a non lordotic cervical spine and increased ROM. Conclusion: Development of degeneration at the level adjacent to region anterior cervical discectomy and fusion performed is higher compared to non-adjacent levels. The level of fusion and the number of fusion levels have no effect on the development of degeneration. (The Me di cal Bul le tin of Ha se ki 2015; 53:120-3)
