Asymptomatic Sinus Bradycardia During Low Dose Cytarabine Infusion In Lymphoma Patient

Özet
Sitarabin lösemi ve otolog kök hücre naklinde kullanılan temel ilaçlardan biridir. Nörolojik, gastrointestinal, hepatotoksik ve myelosupresif yan etkileri daha yaygın olarak görülmesine rağmen, kardiyak yan etkileri nadir ve genellikle yüksek doz 1-3 gr/m tedavi ile ilişkilidir. Bu yazıda, düşük doz sitarabin infüzyonu sırasında asemptomatik sinus bradikardisi gelişen lenfoma vakasını sunuyoruz. Vaka: 41 yaşında erkek hasta refrakter lenfoma nedeni ile düşük doz 100 mg/ sitarabin içeren otolog kök hücre nakli tedavisine alındı. Sitarabin infüzyonunun 3. Gü- nünde asemptomatik sinus bradikardisi gelişti. Tüm diğer nedenler dışlandı ve ilaç infüzyonu durduruldu. Tedavinin kesilmesi ile birlikte kalp hızı normal sınırlara döndü. İnfüzyonun tekrar başlanması ile birlikte bradikardi tekrarladı. Tedavi yakın takip ve monitorizasyon ile sorunsuz tamamlandı. Sonuç: Bradikardi düşük doz sitarabin tedavisi sırasında nadir görülmesine ragmen, klinisyenler bu yanetkiyi bilme- li ve tedavi yakın vital takibi ve gerekirse monitorizasyon eşliğinde verilmelidir.
Abstract
Cytarabine, analogue ofcytidine, is the mainstaytreatment in acute leukemias and autologous stem cell transplantation (ASCT) in lymphoma patients. Although neurologic, gastrointestinal, hepatotoxic and myelosupressive side effects are common, cardiovascular side effects are rare and mostly associated with high doses(1-3 g/m2). Herein, we report lymphoma patient experienced asymptomatic sinus bradycardia during low dose cytarabine infusion. 41 years old male patient with refractory lymphoma was treated with conditioning regimen consisted of low dose(100 mg/m2) cytarabine for autologous stem cell transplantation. 0n the third day of infusion, he developed asymptomatic sinus bradycardia. All the other reasons were excluded and after the cessation of the drug, the rate returned to normal range. Bradycardia recurred after initiation of the infusion. The treatment was completed under close monitoring. Although bradycardia is very rare especially in low doses clinicians must aware of this and close monitoring should be performed during the infusion.
