Postpartum Dönemde Kadınların Psikososyal Durumları ve Etkileyen Risk Faktörleri

Özet
Postpartum depresyon doğum komplikasyonlarından biri olup, uzun yıllar sürebilir hatta doğum sonrası psikoza dönüşebilir. Bu nedenle doğum sonrası dönemde psikolojik reaksiyonların uygun ölçeklerle değerlendirilmesi, önleyici ve tedavi edici yaklaşımlar büyük önem taşımaktadır. Bu çalışmanın amacı, postpartum dönemde psikiyatrik ölçeklerin kullanıldığı makalelerinin incelenmesidir. Google Akademik ve Pubmed veri tabanları kullanılarak Türkiye'de yapılan, 69 makaleye ulaşılmış, 31 araştırma çalışma konusu ile ilgili olmadığı için araştırma dışında bırakılmıştır. Bu derlemede Türkiye'de yapılmış ve 2010-2016 yılları arasında basılmış 15 Türkçe, 23 İngilizce makale incelenmiştir. Çalışmalarda, Edinburgh Doğum Sonrası Depresyon Ölçeği (n=32), Beck Depresyon Ölçeği (n=4), Doğum Sonrası Depresyon Ölçeği (n=2), Hamilton Depresyon Ölçeği (n=1), Doğum ve Doğum Sonrası Döneme İlişkin Endişeler Ölçeği (n=1), Hastane Anksiyete ve Depresyon Ölçeği (n=2), Durumluluk ve Süreklilik Kaygı Envanteri (n=3), Çok Boyutlu Algılanan Sosyal Destek Ölçeği (n=7), Maternal Bağlanma Ölçeği (n=2), kullanılmıştır. Bu çalışmada, doğum sonrası anksiyete, depresyon prevalansı ile risk faktörleri incelenmiş ve Türkiye'de postpartum depresyon prevalansının %15.4-51.3 arasında değiştiği saptanmıştır. Doğum sonrası depresyonu değerlendirmede çeşitli ölçekler olmasına rağmen en sık Edinburgh Doğum Sonrası Depresyon Ölçeği kullanılmıştır
Abstract
Postpartum depression is one of the complications seen after birth which may continue throughout years and potentially may lead to psychosis after delivery. Hence, it is very important to determine, evaluate, and prevent woman’s psychiatric reactions during postpartum period using specific scales. The aim of this study was to review the psychological scales used in studies evaluating depression during postpartum period. Among 69 accessed from PubMed and Google Academic database, 31 papers excluded after reviewing their irrelevance to the subject of the investigation. A total of 15 papers in the Turkish, and 23 in the English language published between 2010-2016 were reviewed. The Edinburgh Postnatal Depression Scale (n=32), the Beck Depression Inventory (n=4), the Postpartum Depression Screening Scale (n=2), the Hamilton Depression Inventory (n=1), the Fear of Childbirth and Postpartum Period Questionnaire (n=1), the Hospital Anxiety and Depression Inventory (n=2), the Spielberger Trait-State Anxiety Inventory (n=3), the Multimensional Scale of Perceived Social Support (n=7) and the Maternal Attachment Inventory (n=2) were used in studies. In this study prevalence and the risk factors for postpartum depression and anxiety were analyzed. In Turkey, the prevalence of postpartum depression was detected to range between 15.4%, and 51.3%. Although there are various scales in order to assess postpartum depression,most frequently the Edinburgh Postnatal Depression Scale was used
