Neovasküler glokomda tedavi seçenekleri ve etkinliklerinin değerlendirilmesi

Özet
Amaç: Neovasküler glokom (NVG) tanısıyla kliniğimizde takip edilen hastaların demografik özelliklerini belirlemek, etiyo- lojide rol oynayan faktörleri ve uygulanan tedavi yöntemlerinin göz içi basıncına (GİB) etkisini araştırmak. Gereç ve Yöntem: Doksan altı NVGli hastanın 99 gözü çalışma kapsamına alındı. Hasta dosyalarından hastaların demog- rafik özellikleri, etiyolojide rol oynayan faktörler ve uygulanan tedavi yöntemlerinin sonuçları retrospektif olarak incelendi. Bulgular: Çalışmaya dahil olan hastaların yaş ortalaması 65.8±7.8 yıl ve ortalama takip süreleri 33.6±6.4 ay idi. Neovas- küler glokoma 54 gözde (%54.54) diyabetes mellitüs (DM), 28 gözde (%28.28) santral retinal ven tıkanıklığı (SRVT), dokuz gözde (%9.09) travma ve sekiz gözde (%8.08) üveitin neden olduğu izlendi. Elli sekiz göze (%77.33) siklokriyoterapi ve postekvatoriyal kriyoterapi, 11 göze (%14.66) mitomisin-Cli trabekülektomi (MMC trab), beş göze (%6.66) intravitreal anti- VEGF enjeksiyonu sonrasında MMC trab ve bir göze (%1.33) Ahmed valv implantasyonu operasyonu uygulandı. Uygulanan tüm prosedürlerdeki ortalama GİB düzeyindeki azalma istatistiksel olarak anlamlıydı (p<0.05). Sonuç: Neovasküler glokomun etiyolojisinde DM ve SRVT önemli rol oynamaktadır. Tedavide uygulanacak MMC trab ile hastaların çoğunda fayda sağlanabileceği düşünülmektedir. MMC trab öncesi uygulanan intravitreal anti-VEGF enjeksiyo- nu ise cerrahi başarıyı arttıran etkin bir tedavi yöntemidir.
Abstract
Purpose: To determine the demographic characteristics of the patients that were followed in our clinic with the diagnosis of neovascular glaucoma (NVG), to investigate the factors that have role in the etiology and the effects of treatment modalities on intraocular pressure (IOP). Materials and Methods: Ninety-nine eyes of 96 patients with NVG included in the study. The demographical character- istics of patients, factors that have a role in the etiology and the results of the treatment methods were analyzed from the patients files retrospectively. Results: Of the patients included in the study, the mean age of the patients was 65.8±7.8 years and the mean follow-up time was 33.6±6.4 months. Diabetes mellitus (DM) in 54 eyes (54.54%), central retinal vein occlusion (CRVO) in 28 eyes (28.28%), trauma in nine eyes (9.09%), and uveitis in eight eyes (8.08%) were found to be the reason for the NVG. Postecva- torial cryotherapy and syclocryotherapy were performed to 58 eyes (77.33%), trabeculectomy with mitomycin C (MMC trab) was performed to 11 eyes (14.66%), MMC trab after intravitreal anti-VEGF injection was performed to five eyes (6.66%), and Ahmed valve implantation was performed to one eye (1.33%). The reduction of mean IOP for each procedure was statisti- cally significant (p<0.05). Conclusion: DM and CRVO have an important role in the etiology of NVG. It is thought that MMC trab performed in the treatment of this disorder can cause benefit in most of the patients. Intravitreal anti-VEGF injection before MMC trab is an efficient treatment modalitiy to enhance the surgical success.
