Özet

Ventilatörle ilişkili pnömoniler (VİP), yoğun bakım üniteleri (YBÜ)'nde en sık görülen infeksiyonlardır. Bu çalışmada YBÜ'de gelişen VİP insidansının, risk faktörlerinin, etken mikroorganizmaların ve duyarlılık paternlerinin belirlenmesi amaçlanmıştır. Hastanemizin dört YBÜ'sünde entübasyon uygulanan hastalar bir yıl boyunca "Centers for Disease Control and Prevention (CDC)" tanı kriterlerine göre prospektif olarak değerlendirildi. YBÜ'de yatan tüm hastalar için demografik verileri ve risk faktörlerini belirlemek üzere form dol¬duruldu, istatistiksel değerlendirmeler ki-kare ve Mann-Whitney/U testi kullanılarak yapıldı. Çalışmaya 147 hasta alındı, 73 (%49.6)'ünde VİP gelişti. VİP gelişen hastaların %59'u erkekti, VİP oranı 16.1/1000 ventilator günü olarak bulundu. Hastalardan 65 (%44. l)"ınde eşlik eden infeksiyonlar mevcuttu. Lojistik regresyon analizi yapıldığında "Acute Physiology Assessment and Chronic Health Evaluation (APACHE)" II skoru, bakteremi, cerrahi alan infeksiyonu, antibiyotik kullanımı, entübe izleniş süresi VİP gelişimi için bağımsız risk faktörü olarak belirlendi. En sık izole edilen etkenler; Klebsiella oxytoca, Pseudomonas aeruginosa, Acinetobacter spp., metisiline dirençli Staphylococcus aureus (MR.SA) idi. izole edilen etkenlerden P. aeruginosa ve Acinetobacter spp. suşlarında karbapenem ve kinolon direnci yüksek bulundu.

Abstract

Ventilator associated pneumonia (VAP) is the most common infection in intensive care units (ICUs). The aim. of this study is to determine the VAP incidence, risk factors, causative agents and the susceptibility patterns. Intubated patients in the four ICUs in our hospital were evaluated prospectively for one year, according to the CDC criteria. Demographic data and risk factors of all the patients in ICUs were collected by filling up a form. Chi-square and Mann-Whitney/U tests were used for statistical analysis. A hundred and forty-seven patients were included in the study. Seventy-three (49.6%) of the patients were diagnosed as VAP.Fifty-nine -percent of these patients were male. VAP ratio was calculated to be 16.1/1000 ventilator days. Sixty-five (44.1 %) of the patients had concomittant infections. Acute Physiology Assessment and Chronic Health Evaluation (APACHE) II score, bacteremia, surgical wound infections, antibiotic usage, duration of intubation were determined as independent risk factors by logistic regression analysis. The most common causative agents were Klebsiella oxytoca, Pseudomonas aeruginosa, Acinetobacter spp. and methicillin resistant Staphylococcus aureus. Carbapenem and quinolone resistance were common among P. aeruginosa and Acinetobacter spp.
