Hastane kökenli stafilokok suşlarında mikrodilüsyon yöntemi ile antibiyotiklere direncin belirlenmesi

Özet
Hastanede yatan hastaların balgam, burun sürüntüsü, idrar, pil ve'yara materyali gibi çeşitli örneklerinden izole edilen 42 'si Staphylococcus aureus ve 33 'ü koagulaz negatif stafilokok (KNS) olmak üzere toplam 75 stafilokok susu çalışma kapsamına alınmıştır. Antibiyotik direnci disk difüzyon ve mikrodilüsyon yöntemleri ile incelenmiştir. S.aureus suşlarında metisilin direnci % 14 iken KNS'larda % 30 olarak saptanmıştır. MİK değerlerine göre metisiline duyarlı S.aureus ve KNS suşlannın antibiyotik direnci sırası ile penisilin G için % 83 ve % 61; ampisilin-sulbaktam için % 14ve% 9; sefalotin için % 8 ve % 13 olarak bulunmuştur. Metisiline duyarlı ve dirençli suşlarda sırası ile trimetoprim-sulfametoksazol için % 10 ve % 56; eritromisin için % 8 ve % 63; klindamisin için % 12ve% 50; gentamiein için % 14ve% 81; ofloksasin için %7ve% 38; siprofloksasin için %7ve%25 direnç saptanmıştır. Sonuç olarak nozokomial infeksiyonlarda metisiline dirençli stafllokoklar antibiyotiklere metisiline duyarlı stafilokoklardan daha dirençli bulunmuştur (p<0.05).
Abstract
In this study, a total of 75 Staphylococcus strains [42 S.aureus and 33 coagulase negative Staphylococcus (CNS)] were isolated from different clinical specimens (sputum, nasal swabs, urine, pus and wound materials) obtained from hospitalized patients. Antibiotic resistance was determined by microdilution and disk diffusion methods. Methicillin resistance was found in 14 % and 30 % ofS.aureus and CNS strains, respectively. According to the MIC values, resistance rates of methicillin sensitive S.aureus and CNS strains for penicillin, ampicillin-sulbactam and cephalotin were 83 % and 61 %, 14 % and 9 %, 8 % and 13 %, respectively. Resistance rates of methicillin-sensitive and methicillin-resistant strains were found to be 10 % and 56 % for trimethoprim-sulfamethoxazole, 8 % ve 63 %for erythromycin, 12 % and 50 %for clindamycin, 14 % and 81 % for gentamicin, 7 % and 38 %for ofloxacin, 7 % and 25 %for ciprofloxacin, respectively. In conclusion, resistance rates to all antibiotics were higher in methicillin-resistant isolates than methicillin-sensitive isolates (p<0.05).
