SON 7 YIL İÇİNDEKİ PAROTİS KİTLESİ HASTALARIMIZIN DEĞERLENDİRİLMESİ: 211 HASTANIN RETROSPEKTİF ANALİZİ

EVALUATION OF THE PATIENTS WITH PAROTIS MASS IN THE LAST 7 YEARS: RETROSPECTIVE ANALYSIS OF 211 PATIENTS

Özet

Amaç: Parotiste kitle tanısı ile opere edilmiş hastalarda endikasyonları, yapılan cerrahiyi, postoperatif tanıyı ve komplikasyonları retrospektif olarak değerlendirmeyi amaçladık. Yöntemler: Hastanemizde 2011-2018 yılları arasında parotidektomi yapılan olgular çalışmaya dahil edildi. Hastaların endikasyonları, yaş ve cinsiyet dağ ılımı, histopatolojik sonuçları ve cerrahinin komplikasyonları değerlendirildi. Bulgular: Kliniğimizde parotidektomi yap ılan 211 olgu çalışmaya dahil edildi. Hastaların ortalama yaş ı 47.64±15.8 (7-82) idi. Ince iğne aspirasyon biyopsi (İİAB) sonuçlarında 172 (%81.52) benign, 29 (%13.74) malignite şüphesi ve 10 (% 4.74) malign olgu, cerrahi patolojilerinde 178 (%84.36) benign, 33 (%15.64) malign olgu mevcuttu. Tüm parotis kitlelerinde en sık karşılan lezyon pleomorfik adenomdu (%43.60). En sık görülen benign lezyon pleomorfik adenoma (%51.68), en sık malign tümör mukoepidermoid karsinomdu (%27.27). Tümör hastaların 100 (%47.39)’ünde sağ, 111 (%52.60)’ inde sol yerleşimliydi. 31 (%14.69) hastada derin lob yerleşimi veya derin lob uzanımı mevcuttu. Kitlelerin 180 (%85.31) i yüzeyel lob yerleşimliydi. En sık uygulanan cerrahi süperfisiyal parotidektomi (%84.36) idi. Malignite nedeniyle opere edilen 25 (%11.85) hastaya eş zamanl ı boyun diseksiyonu yapıldı. Hastaların %30’unda komplikasyon görüldü, en sık görülen komplikasyon fasiyal paralizi idi. Sonuç: Parotis tümörlerinin büyük çoğunluğu benigndir ve tercih edilen tedavi modalitesi cerrahidir. En sık görülen benign patoloji pleomorfik adenomdur. Süperfisial parotidektomi en sık uygulanan cerrahidir. Parotis cerrahisinde en sık karşılaşılan komplikasyon fasiyal paralizidir. Preoperatif histopatolojik tanı, kitlenin yerleşimi, tümörün evresi, fasiyal sinir invazyonu ve boyunda palpable lenf nodu varlığı gibi etkenler uygulanacak cerrahinin planlanmasında önemli rol oynar. Malign parotis tümörlerinde gerektiğinde boyun diseksiyonu tedaviye eklenmelidir.

Abstract

Objective: We aimed to evaluate the indications, surgical procedure, postoperative diagnosis and complications in patients with parotid mass retrospectively. Methods: The patients who underwent parotidectomy between 2011-2018 were included in the study. The indications, age and sex distribution, histopathological results and complications were evaluated. Results: A total of 211 patients who underwent parotidectomy were included in the study. The mean age was 47.64 ± 15.8 (7-82) years. The results of fine needle aspiration biopsy (FNAB) revealed 172 (81.52%) benign, 29 (%13.74) malignancy suspicion, and 10 (% 4.74) malignancy, 178 (84.36%) benign and 33 (15.64%) malignant in surgical pathology. The most common lesion in all parotid masses was pleomorphic adenoma (43.60%). The most common benign lesion was pleomorphic adenoma (51.68%) and the most common malignant tumor was mucoepidermoid carcinoma (27.27%). The tumor was located at right side in 100 (47.39%) and at left side in 111 (52.60%) patients. 31 (14.69%) patients had deep lobe localization or deep lobe extension. 180 (85.31%) of the masses were located in the superficial lobe. Superficial parotidectomy was the most common surgical procedure (84.36%). The concurrent neck dissection was applied to 25 (11.85%) patients. The complications were seen in 30% of the patients and the most common complication was facial paralysis. Conclusion: The majority of parotid tumors are benign and the most common one is pleomorphic adenoma. Superficial parotidectomy is the most frequently used surgical procedure and its common complication is facial paralysis. Preoperative histopathological diagnosis, location of the mass, tumor stage, facial nerve invasion and the presence of palpable lymph node on the neck play an important role in the planning of the surgery.. In cases of malignant parotid tumors, neck dissection should be added to the treatment.
